32 | reviewed the following documents: children's roster, staff roster, children’s sign-in & sign-out sheets (9/29/2025 to 10/02/2025), staff schedule, staff timesheets (9/29/2025 to 10/02/2025), facilities log (maintenance) and outgoing notifications. Additionally, LPA initiated staff interviews.
On 11/21/2025 LPA concluded interviews with all the relevant parties.
Based on observation, interviews and record review, no information revealed that staff did not ensure toilets were working properly. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.
An exit interview was conducted with Director Teresa Antonelli. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
