On 12/09/2025 Licensing Program Analyst (LPA) Cristina Castellanos made an unannounced visit to the above-mentioned facility for the purpose of delivering complaint findings. Upon arrival, LPA met with Director Teresa Antonelli and stated the purpose for the visit.

The investigation of the above-mentioned allegation was conducted by LPA Castellanos.

During today’s tour of the facility LPA observed the following: 20 Infants in care with 8 staff members & 36 toddlers with10 staff members. In total there were 56 children in care with 18 staff members providing care and supervision.

reviewed the following documents: children's roster, staff roster, children’s sign-in & sign-out sheets (9/29/2025 to 10/02/2025), staff schedule, staff timesheets (9/29/2025 to 10/02/2025), facilities log (maintenance) and outgoing notifications. Additionally, LPA initiated staff interviews.

On 11/21/2025 LPA concluded interviews with all the relevant parties.

Based on observation, interviews and record review, no information revealed that staff did not ensure toilets were working properly. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.

An exit interview was conducted with Director Teresa Antonelli. A copy of this report and appeal rights were discussed and left with the Director. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
