25 | Licensing Program Analysts (LPA) Laticia Thompson conducted an unannounced case management inspection on 07/13/2023 due to incidents that were reported to the department. LPA met with Tiara Stevens, Business Manager who guided LPA on a tour of the facility. There were 99 children with 15 staff present upon arrival.
LPA conducted interviews and obtained documentation by email.
One incident occurred on 06/01/2023 and was reported to the Department on 06/07/2023, via telephone. The facility failed to report the incident that occurred on 06/01/2023 within the 24 hours of occurrence. The second incident occurred on 06/07/2023, facility left a voicemail message on 06/08/2023 and the department returned the call on 06/09/2023. The facility reported the Unusual Incidents to the Department that occurred on 06/07/2023 within the required 24 hours. Information reported to the Department indicated that 2 incidents occurred that pertained to health and safety.
LPA conducted a walk through to ensure that the health, safety, and personal rights of children in care are being met.
There were no deficiencies cited during today’s inspection.
The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. An exit interview was conducted with Teresa Antonelli, Director, including, but not limited to Provider Rights, Appeal Procedures.
