Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 year inspection. LPA met with Knar Nazlikian, Director, who guided analyst on a tour of the facility. This is a preschool program serving up to 130 children. The facility's hours of operation are from 8:00 AM to 5:00 PM. The facility is currently utilizing 10 classrooms. The facility consists of ten classrooms, Director's office, and a play yard. Director's office used as isolation area for sick children. LPA observed 97 children in care being supervised by 17 staff members (8 teachers and 9 aides) during this inspection. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA reviewed the sign in and sign out sheet to verify the census. Facility is operating within capacity limitations. LPA reviewed the sign in and sign out records to verify the census. LPA observed 13 children in Room A1 supervised by two staff, 11 children in Room A2 supervised by two staff, 12 children in Room A3 supervised by two staff, 13 children in Room B1 supervised by two staff, 13 children in Room B2 supervised by two staff, 11 children in Room B3 supervised by two staff, 11 children in Room C1 supervised by two staff, and 13 children in Room C2 supervised by two staff. There are no bodies of water on the premises during the visit. LPA observed the following postings by the Director's office: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269). Furniture and equipment were inspected for age appropriateness and good repair. The classrooms were inspected to ensure that the floors have a surface that is safe and clean.

The bathrooms and toileting areas were inspected, there are sufficient toilets and sinks to accommodate the facility’s capacity. Toilets flush properly, toilet and sinks are reachable by the children in care. Each restroom has adequate toilet paper and paper towels available. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom areas.

The facility provides two snacks and lunch. Menus are posted. There is a full service kitchen on site. The kitchen is equipped with a refrigerator, freezer, stove, sink and microwave oven. The kitchen has cold and hot running water. The kitchen is off limits to children in care. There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom. Drinking water is available both indoors and outdoors via pitchers. There are first aid supplies in each classroom and Director's office. Children are inspected for illnesses as they arrive. There is a separate area for isolation and care of ill children in Director's office. There is a cot available for each ill child. An isolated toilet and sink are available for ill children. Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. LPA observed cots available for each child. Cots are being sanitized once a week. Bedding is stored separately. Napping children are properly supervised within ratios.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. The facility has seven functioning fire extinguishers (serviced on 10/17/2024), smoke and carbon monoxide detectors that meet statutory requirements. Facility was observed to be operating within the conditions, limitations, and capacity specified on the license. Staff trained in CPR and Pediatric/Infant First Aid. Medication and Discipline policies were discussed. LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights on file. The children's roster was available and current.

Staff records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. Staff do not have current Mandated Reporter Training Certificates, two staff do not have Health-Screening reports in staff files, two staff do not have proof of education credits.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
