On 6/8/22 Licensing Program Analyst (LPA) Doris Whitmore, conducted an unannounced Annual Required Inspection for the state preschool license. At the time of inspection there were a total of 12 children. LPA observed a Teacher and a Teacher Aid in the classroom. LPA met with Ernestine Jauregui. and toured the facility indoors and outdoors.

This is a state preschool located on the Grounds of Osceola Elementary School. Days of operation are Monday – Friday the facility offers morning and afternoon. Morning care is from 8:00a.m. a.m. to 11:00a.m. and afternoon care is from 11:30 a.m. to 3:00p.m. The children do not take naps in the program. Morning care children have snack and take their lunch home. The afternoon care children eat lunch on campus.
First Aid supplies, smoke detectors, and fire extinguishers were observed and in operable condition. In the classroom currently there is no Carbon Monoxide. Trash cans with tight lids were observed. The center provides food for the children enrolled. Food is prepared by the Los Angeles Unified School District and brought in daily Menu is posted for parents’ review


There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection. Page 1

Furniture& Playground and equipment are in good condition, free of sharp, loose or pointed parts. The play yard is located directly outside the back entrance of the classroom and is securely fenced. Equipment was inspected for safety, good repair and age appropriateness. Large climbing structures, slides, and other large play equipment on site is securely anchored with cushioning underneath. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin.

All foods/beverages capable of rapid spoiling are stored in covered containers at 45 degrees or less. Solid waste storage containers have tight-fitting covers and are in good repair. Children also bring individually labeled water bottles from home. and no bodies of water observed in the outdoor play area. All Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. No individuals excluded by the Department are allowed to be present. Capacity and limitations as specified on the license are being maintained the name of the childcare center director or fully qualified teacher(s) designated to act in the director’s absence has been reported to the Department. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

LPA reviewed a sample of ten children’s files All of the files were missing the Personal Rights& Notification of Parents Rights. LPA spoke with the Principal and Office Tech. Staff Files will be reviewed later and are off site.Incidental Medical Services (IMS) are currently not being provided being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
