On 03/11/2025, Licensing Program Analyst (LPA) Lisa Clayton conducted an unannounced Required Inspection at Gan Malibu EEC, Infant Center. LPA Clayton met with Director Sarah Cunin. LPA Clayton observed 7 infants being supervised and cared for appropriately by 4 fingerprint cleared staff. Facility hours of operation are Monday – Friday, 8:00am – 5:00pm. The CCC provides water.

LPA Clayton toured the facility indoors and outdoors for a Health and Safety Inspection. Capacity and limitations as specified on the license are being maintained.

All children are under visual supervision of a teacher at all times. The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios.

There are no pools, ponds, Jacuzzis, spas or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked cabinet area.

LPA Clayton inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

LPA Clayton observed changing station to be clean, have raised sides at least 3 inches high and within arm’s reach of a sink. The sleeping area for infants is physically separate from the indoor activity space. LPA Clayton inspected napping equipment placement and bedding for safety.

All hand washing facilities and toilets are in safe and sanitary operating condition. Water temperature, toilet paper, paper towels, safety and sanitation were inspected. Floors in the facility are clean and safe.

All meals are provided by the parents/authorized representative. The food storage area is free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair.

LPA Clayton observed a fully charged 2A10B:C Fire Extinguisher in the infant room. LPA Clayton instructed Director to purchase a carbon monoxide detector for the infant license/room. The facility has a pull fire alarm in the preschool class and LPA Clayton observed a centralized fire alarm system. Director provided an inspection report from the County of Los Angeles Fire Department dated 01/16/2024.

Director Sarah printed a Sleep Log from the BrightWheel app and LPA observed staff documentation of physically checking on the infants every 15 minutes, and documenting sleep position. LPA Clayton provided Director with an Infant Needs and Services Plan sample to be completed for all infants in care and all newly enrolled infants prior to their presence in the CCC.

LPA Clayton reviewed 5 infants files and observed the files to be in compliance including but not limited to Individual sleep plans, current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent’s Rights and signed Personal Rights receipts.

LPA Clayton reviewed 5 employee files which were found to be in compliance with immunization for childcare providers and CPR/First Aid as required for opening and closing staff, Mandated Reporter training certificates.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Incidental Medical Services – IMS are not currently being provided in the infant program. LPA Clayton observed the area were medication is stored. LPA Clayton provided Director with a copy of the IMS Sample plan form for her reference. For IMS information see PIN 22-02-CCP. Director Sarah was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Clayton reminded the staff to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Childcare, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies were cited, Technical Advisory was issued (see LIC 9102).

During the exit interview the report was reviewed and a copy was provided to Director Sarah.
