25 | Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced inspection on 11/20/2024. The purpose of the visit was to ensure that the health, safety, personal rights, licensing conditions and limitations are as required by Title 22 Regulations. LPA arrived at the facility at 8:45AM and met with Maria Gloria Sanchez, Assistant #1, who guided LPA on a tour of the facility. Also present was Assistant #2. LPA was later met by Sheryl Matheu, Licensee.
Sheryl Matheu, Licensee, is licensed for a capacity of 14 children. LPA observed 11 children in care. Licensee had 1 infant and 10 preschool age children in care upon arrival.
On 10/24/2024, a noncompliance conference was called by the Department to discuss with Licensee the deficiencies of operating over capacity and out of ratio. Licensee was advised to provide a copy of the report received during the noncompliance conference to the parent/guardian of children currently enrolled by the next business day or immediately upon return. Licensee was to obtain signature and date from the child's parent/guardian on the Acknowledgement of Receipt of Licensing Reports (LIC 9224). Licensee was to keep a record immediately upon receipt of the completed and signed LIC 9224 acknowledging receipt of this report in the child's file.
During todays visit, LPA review child files to verify that the LIC9224 were completed and signed by each child's parent/guardian. LPA observed completed and signed LIC9224 in the children's files reviewed indicating noncompliance conference report was provided.
There were no deficiencies cited during today's inspection.
The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
