On 7/18/2023 Licensing Program Analyst (LPA), Suzette Ornelas conducted unannounced required annual inspection at the Van Nuys Preschool (FAC 197420046). LPA met with the Assistant Michelle Gonzalez and Center Director, Youngsun Moon and discussed the purpose of the visit. The Assistant guided LPA on a tour of the facility inside and out, Director arrived and concluded facility tour. The facility is located inside of the premises of the Van Nuys United Methodist Church. The facility uses the first floor of the Church Education building. The facility is currently using four classrooms.

LPA observed all required postings in a prominent area. A review of the sign in/out sheet was conducted to verify the current census of children. LPA observed 42 children and 7 adults. All children are under supervision, including visual supervision, of a teacher at all times. Capacity and limitations as specified on the license are being maintained. A walk through of the classroom area was conducted. LPA observed 6 licensed preschool classrooms on the premises, 1 which is a toddler classroom.

All areas identified on the Facility Sketch were inspected. The following staff were present during this inspection:
Toddler Room: 6 toddlers with a ratio of 2 teacher; this classroom has a changing table.
Blue Room: 15 preschool children with a ratio of 2 teacher
Green Room: 12 preschool children with a ratio of 1 teachers; the kitchen is accessed through this classroom through a locked door.
Yellow Room: preschool children with a ratio of 1 teacher; this classroom has a toilet, sink, and changing table.

A walk through of the classroom room space was conducted, the space was found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate.

There is adequate heating, lighting and ventilation. Chipped paint was observed throughout the facility walls, per Director, painting will take place during the week of 7/30/2023, when children are not present. Isolation area for sick students is in the Director’s office. LPA observed adequate arrangements for isolation and care of ill children.

The bathroom and toileting areas were inspected, LPA observed toilets and sinks to accommodate the facility’s capacity. Facility is equipped with 5 toilets and 3 sinks. Toilets flush properly, toilet and sinks are reachable by the children in care. The restrooms have adequate toilet paper and paper towels available. The bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom area.



Playground equipment is in safe condition and free of hazards. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. LPA observed a separate play yard for both toddlers and preschool children. The wood chip outdoor play yard is not currently being used. All kitchen, food preparation and storage areas are clean and safe. Per Director, school provides daily meals for children for children who are enrolled in the food program. Parents provide daily meals for children who are not enrolled in the food program. Snacks are provided for all children in care. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is readily available both indoors and outdoors. The facility is equipped with a working smoke detector and carbon monoxide detector. LPA observed fire extinguishers (2A 10BC) located throughout the facility with service date of 8/24/2022. There is no swimming pool or other bodies of water on the premises. First Aid Kit was observed to be complete. Fire Drill log was observed to be complete and current. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. No poisons were observed during the inspection.

The facility has sufficient cots or mats for napping. Each mat or cot is occupied by only one child at time. LPA reviewed a sample of children’s files and observed files were complete. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff files and observed files were complete. Pg2

LPA discussed the safe sleep regulations and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA provided director with infant safe sleep regulation, sleep log sample, safe sleep frequently asked questions and safe sleep flyer. Provider will implement safe sleep documentation immediately. LPA Provided Director w/ copy of the Preschool Program w/Toddler Component Regulation and information on ensuring the facility follows regulating requirements, to be maintained at the facility moving forward. LPA provided Director with a copy of the Disaster and Mass Casualty Plan regulation and information on ensuring the plan follows regulating requirements, to be maintained at the facility moving forward.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA referred Director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
