25 | On 10/12/2023 at 8:52am, Licensing Program Analyst (LPA) Andrea Pittman conducted an unannounced case management visit at the facility and was met by Site Supervisor Noelle Heflebower who permitted entry to the facility. LPA toured the facility with the Site Supervisor according to the facility sketch. Upon arrival, LPA observed 21 children with 2 staff members providing care and supervision to the classroom.
On 8/11/2023, the Site Supervisor Noelle Heflebower submitted a LIC 624- Unusual Incident Report stating that an incident occurred on 8/9/2023 in the preschool classroom involving Child 1 attempting to initiate inappropriate contact with Child 2. After observations, interviews, and record reviews, the following information was revealed about the incident resulting in the submission of the LIC 624-Unusual Incident Report:
During this investigation, LPA received pertinent documents related to this investigation, which included the facility’s staff and children’s rosters, reports, and other relevant investigation documents. It was revealed that Staff 1 was folding mats and transitioning the children to the sitting area. Staff 2 was helping the children and putting away mats, as well. Child 1 was standing over Child 2 in the carpet area of the classroom. When Child 1 made an inappropriate comment and gestured to Child 2. Staff 1 overheard the statement made by Child 1 and spoke with Staff 2 to ensure that they heard Child 1 correctly. Staff 1 then called Child 1 over and confirmed the inappropriate comment made by Child 1 and provided guidance to Child 1. Staff 1 notified the parents of Child 1 of the incident and completed the proper cross reporting. Additionally, the Director reported the incident to Licensing. LPA provided technical assistance with the regulations for 100% supervision and reporting requirements.
As a result, there are no citations being cited at this time. The center took proper action in protecting the children in care and properly reporting.
An exit interview was conducted, and a copy of this report was provided to Site Supervisor along with the Notice of Site Visit and Appeal Rights.
