13 | On October 20, 2021 at 2:46pm, Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced complaint subsequent inspection on the above allegation. LPA disclosed the purpose of inspection and was permitted entry by Director who guided LPA on tour of the facility. LPA counted 7 children with 2 staff present.
During this investigation, LPA interviewed Director, Staff, children and obtained facility records. Staff records were observed and found to be in compliance. LPA observed staff rotating and communicating during breaks and lunch, children stayed within age groups and observed. Interviews and record reviews did not disclose unqualified staff, administrator/director working with children.
Based on evidence obtained and interviews conducted, the above allegation is deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations did or did not occur. An exit interview was conducted, a copy of this report, Appeal Rights, and a notice of site visit were provided to the Director Shirley Johnson. |
