On 11/08/2023 at 08:54 a.m., Licensing Program Analysts (LPA) Cristina Castellanos and Licensing Program Manager (LPM) Claudia Escobedo conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 8/11/2023. Upon arrival, LPA met with Dr. Joanna Clifton, Director and explained the purpose of the visit. LPA and LPM toured the facility inside and out.

There were 46 children present at the facility and 4 Teachers and 11 Instructional Aides.

According to the UIR, on 8/11/2023, the children named in the report eloped from the group outside, to the rear classroom door.

During today’s inspection, LPA interviewed staff and reviewed records. There were no citations issued. The UIR investigation will continue.

An exit interview was conducted. A copy of this report (LIC 809) and Notice of Site Visit was provided.
