This report is being amended due to lead water testing not being completed. LPA amended page 5, closing scripts.
On April 1, 2026 at 1:12 P.M., Licensing Program Analyst (LPA) Crystal Ali met with the facility Licensee, Maria Valdovinos and Assistant Supervisor, Blanco Quintero to conduct an unannounced annual inspection for the Preschool license in accordance with the facility sketch. A tour of the facility was conducted. LPA observed a total of 29 preschool children for classrooms (Blue Room for 3’s and Green Room for 2’s) with the director and three staff providing care and supervision. The hours of operation are 6:30 A.M. to 6:30 P.M., Monday - Friday. Incidental Medical Services (IMS) was discussed.
Indoor/Children’s Area:
Child care center is clean, safe, sanitary and in good repair; floors of all rooms have a surface that is safe and clean, cleaning compounds are inaccessible, poisons are locked, furniture/equipment is in good condition. Indoor activity space for preschool children is physically separate from space used by children in infant/toddler components. Preschool area is equipped with age-appropriate furniture and equipment including tables, chairs, and cubbies to meet children’s needs; Per Assistant Supervisor, the tables, chairs, and cubbies are sanitized every day when children are napping. Toys are usually sanitized once a week. All play equipment and materials used by children are age-appropriate, each child has an individual or portable storage space (cubby, individually labelled with name) for his/her clothing and personal belongings. The facility has a working telephone.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Trash bins have tight fitting lids. Soiled and wet diapers are placed inside trash bins with lid. Drinking water is available in the preschool rooms. The facility provides disposable cups to children when drinking. LPA observed sufficient quantity of cots available for napping preschool children. All materials and surfaces are toxic free and are inaccessible. There is no fireplace in the facility. The center has an internal pull-down fire alarm system and 2-A:10-B:C type of fire extinguishers (dated 4/11/25). Smoke and Carbon monoxide devices are in operable condition per fire marshal standards. Licensee states that fire marshal came last in September 2025 and replaced the whole system.
Advisory/Other: First Aid kit was observed with supplies readily available. The thermometer is kept separately from the first aid kit. Sign in and out sheets were reviewed. The facility is using manual sign in and out sheet and were recorded daily. The parent board was reviewed and has all the required forms posted. Fire/earthquake drills current, 3/11/26 at 9:46am.
Restrooms: LPA inspected and observed a total 4 toilets and 4 sinks in the bathroom. Staff members have a separate restroom to use. The LPA observed liquid soap, toilet paper, trash bins with covers, paper towels, hand sanitizers, and sanitary wipes were readily available. Water temperature is appropriate. Per Assistant Supervisor, Staff rotate on cleaning the restroom every day during nap time. Staff mop the floors daily at the end of each day and clean table counter tops throughout the day. There is an isolation area in the back of the Green room beside the children restroom and parents are immediately called to pick up their children. The facility maintains a comfortable temperature at all times, first aid supplies, sign in/out sheets are available and completed daily. Smoking is prohibited on the premises, daily inspection for illness, no prohibited child care items observed. Firearms/weapons are not allowed or stored on premises. There is no body of water on the premises.

Napping: LPA observed children's personal blankets. Facility provides sheets for children in care during nap time. Mats are used and maintained in good condition, wiped/cleaned/sanitized, or when soiled and there is no contact with other children bedding. LPA observed nap time.

Outdoor: The facility has a playground area for the infant/toddlers and preschool children. Currently, the preschool children (2’s, 3’s, and 4’s) used the outdoor playground in the morning between 9:00 A.M. to 9:30 P.M. Drinking water is provided during outdoor plays. Outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. LPA observed two canopies at the outdoor playground. The area was observed to be free of debris, free from hazard, holes, broken items, debris. The slides have wood chips to absorb the fall. No bodies of water were observed. LPA observed tricycles handlebars that are missing their cushion. Licensee states she will repair the tricycles handlebars.

Staffing Ratio and Capacity: Teacher child ratios were observed, and staff name recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate.

Health Related Services: Per Assistant Supervisor, children were physically observed when they arrived at the center. Licensee states that there are no children on medications and no children with food allergies. Children are inspected for illnesses (wellness policy) as they arrive.

Food Service: There is a clean microwave, refrigerator, and freezer available in the facility. Facility provides two meals (breakfast and lunch) and three snacks are provided for children in care. The facility does not have a stove. This is not a full kitchen. Staff prepares food that is microwave only. Children can bring their packed lunches or snacks if they choose to. LPA observed gallons of water in the kitchen area for drinking. LPA observed snack time. LPA observed knives in the 2nd drawer to the right of the microwave, which was assessable to children. Licensee immediately moved the knives to the cabinet under the sink with safety latch, making them inaccessible to children.
Staff/Personnel Records: LPA reviewed with facility representative the LIC 125, records to be maintained at the facility, for child’s records, personnel records, administrative records, and parent board. LPA reviewed staff files (3). All staff files are in compliance. LPA reviewed child (4) records. All child records are in compliance.
Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty. The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 393), PUB 269 Child Passenger Restraint System Poster, emergency disaster plan, and earthquake preparedness checklist.

No deficiencies cited in accordance with Title 22 of the California Code of Regulations and Health & Safety codes.

A notice of site visit was given and must remain posted for 30 days. A copy of this report and copy of appeal rights were provided to facility representative. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Criminal Record Clearance - Child Care Centers
Facility representative was reminded that all adults 18 and over, including
employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Lead Testing – Child Care Centers (CCC)
CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE,
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives
outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and
discussed the Child Care Licensing Safe Sleep webpage at
