On 4/19/23, Licensing Program Analyst (LPA), V. Wheatley LPA was greeted by licensee Silvia Sanchez and assistant Miriam Moreno Chavira aka Gisel

LPA observed 8 children napping in playpens and on cots. There is one infant Child #1. LPA observed the children supervised.

During an inspection, LPA toured the outdoor play area in the rear of the home. LPA observed several unsafe items such as several pieces of wood and a pile of metal poles. The metal poles is from workers who replaced the large canopy that was damaged from the wind. In addition, LPA observed the outdoor area accessible to a side area with other unsafe items such as a ladder.

The facility is being cited for a Type A violation.

During the inspection two other children arrived Child #9 and #10.

In addition, LPA reviewed children's records. LPA observed three children (Child #1, #2 and #8) on the premises without immunization records and one child present without a file. The child without a file was enrolled previously.

This is a type B violation.

The licensee understands that the inside and the outside of the home must be safe for the day care children at all times. Licensee will clean the backyard and remove all unsafe items by 4/21/23. LPA will ensure the children's safety at all times. Licensee will submit a Plan of Correction to the Department by sending photos.

102421(b) The licensee shall maintain, in each child's record, the signed and dated notice form required in Section 102419(d).The licensee shall maintain, in each child's record, a copy of the emergency information card as required in Section 102417(g)(7).

The licensee understands that each child must have a file with required emergency information. No child can be enrolled without these required files. The licensee will obtain required forms completed by parent and submit proof of the copies to the Department by 4/21/23. The licensee was reminded the files must be kept 3 years after the child leaves.
