25 | On July 24, 2026, at approximately 1:27pm, Licensing Program Analyst (LPA) Melissa Zaragoza conducted an unannounced, plan of correction (POC) visit at the facility. LPA met with licensee, Fabiola Orellana, and explained the purpose of the visit. Present during today's visit was the licensee, 1 infant, 4 preschoolers, and 2 school age children.
On July 15, 2026, at approximately 10:00am, LPA Zaragoza conducted an unannounced, annual inspection and walk through inspection, and reviewed staff files. During the inspection, the facility was issued 1 Type B deficiency for having an expired Mandated Reporter Training certificate. LPA reminded the licensee, Mandated Report Training certification must be renewed every 2 years, for all staff working with children. Licensee stated, they will make sure they renew their certification and will send LPA proof of completion.
As of this date, 07/24/2026 LPA observed the licensee's file and observed the licensee to have renewed their Mandated Reporter Training certificate. The licensee's Mandated Reporter certificate is valid until 07/16/2028.
Deficiencies cited on July 15, 2026, have been cleared during today's visit. LPA provided the licensee with POCs letter. No deficiencies were cited today. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days.
