On 09/26/2023 LPA Whitmore initiated the complaint investigation and met with Ana Wilford Director LPA toured the facility indoors and outdoors, observing proper teacher/ ratios with a total of 9 children and 5 teachers. LPA interviewed the Director, Owner, Staff & Children. LPA obtained a copy of the Facility Roster, Personnel Report, Discipline Policy, Sign in Sheets, &Contract Agreement.
On 12/14/2023 at 9:30 a.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Director Ana Wilford. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 11 total children in care and 4 Teachers.
The Department conducted a full investigation, which included staff interviews, interviews with relevant parties and other agencies, as well as a record review which included documentation related to the allegation. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegation of Personal Rights- Day care child sustained multiple bruising while in care.Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur. There was not sufficient evidence to prove the bruises

occurred while the child was in care at the facility and the time of day that the incident occurred. With interviews conducted, staff advised that wellness checks of children are conducted daily upon arrival. Therefore, the allegation is deemed unsubstantiated. An exit interview was conducted, a copy of this report, appeal rights along with Notice of Site Visit were provided.
