13 | On 09/26/2023 LPA Whitmore initiated the complaint investigation and met with Ana Wilford Director LPA toured the facility indoors and outdoors, observing proper teacher/ ratios with a total of 9 children and 5 teachers. LPA interviewed the Director, Owner, Staff & Children. LPA obtained a copy of the Facility Roster, Personnel Report, Discipline Policy, Sign in Sheets, &Contract Agreement.
On 12/14/2023 at 9:30 a.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Director Ana Wilford. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 11 total children in care and 4 Teachers.
The Department conducted a full investigation, which included staff interviews, interviews with relevant parties and other agencies, as well as a record review which included documentation related to the allegation. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegation of Personal Rights- Day care child sustained multiple bruising while in care.Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur.
