On 08/04/2022 Licensing Program Analyst (LPA) Doris Whitmore met with Gerald Huang, Administrator for the purpose of an unannounced inspection. The preschool is located on the property of St. Paul Church. The school can be accessed by parking in the rear parking lot of the church. LPA Whitmore toured the facility, all identified rooms per facility sketch was inspected, the following was observed. Pre- school hours are from 7:30am to 6pm Monday through Friday.

Upon arriving at the preschool LPA observed 22 children being supervised by 5 teachers in 3 classrooms. LPA observed sign/sign out sheets on the APP Bright Wheels. Facility is operating within capacity limitations. LPA observed 3 classrooms on the property. LPA observed classroom #1 for ages 3 to 5 years old, classroom #2 for ages 3 to 5 years old and classroom #3 for ages 2 to 3 years old. A walk through of each was conducted, classrooms were found to be clean and free from any potential hazards. LPA observed classrooms to have dramatic play, arts/crafts, reading and learning activities. Wall heaters were observed in each classroom covered for safety.
Furniture in each classroom was found to be in good repair and age appropriate. Individual cubbies were observed for children's belongings. There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom. Administrator states that the isolation area for sick students is located in the Directors Office.. Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. Preschool children nap in individual classrooms.
The bathroom area(s) were inspected, each classroom is equipped with a bathroom, there are sufficient toilets and sinks to accommodate the campus capacity. Toilets flush properly, toilet and sinks are reachable by the children in care. Each restroom has adequate Toilet paper and paper towels available. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom area(s). No hazards were observed. Lunches are not prepared by the school; Parents provide lunch for their children. Preschool provides two snacks daily. Cleaning supplies are out of reach to the children and is stored separately in a locked room outside of classroom.

All snacks are kept in small kitchen next to the Directors Office. LPA observed snack in tight containers and sealed for safety. Kitchen area was observed inaccessible to children, clean, neat and free from hazards.
Inspection of the outdoor play area was conducted. Preschool has two small playgrounds located in the front and back of the school. Play equipment was found to be securely anchored with adequate resilient cushioning material underneath and around the perimeter. Drinking water is readily available on the play yard. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc. Front playground has a garden for children to water plants and vegetables.

Medication is properly labeled and stored in the original container. First Aid supplies are stored in each classroom and in the Director's office. All medication is stored in Director’s office.

LPA reviewed children and staff records for completeness records were found to be complete.
A record of teachers/assistant’s names was check against current facility roster; Staff are currently associated to the campus in which they are assigned to work. LPA observed CPR/First Aid expires 04/23. Mandated Reporter Training competition 04/24/22. The required fire extinguisher, smoke detectors and carbon monoxide detector are in operable condition. LPA observed required postings in the facility (Facility License, Emergency Disaster Plan, Parents Rights Poster. Notification of Parents’ Rights, Personal Rights. Documents reviewed during the inspection were verification of Disaster Fire Drills, Earthquake Preparedness, Daily Activity Schedule posted in each classroom.

The following was discussed with the Administrator:


Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

The ADA requires that child care providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services.



Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents.

The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

The Administrator was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).
An appeal may be filed, in writing 15 business days from the date of receiving the penalty assessment.

The Administrator was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit. New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.


New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

Incidental Medical Services: Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility’s Plan of Operation and submit an updated Plan of Operation to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.
