32 | According to Staff interviewed during the course of the investigation, there was a substitute teacher assisting one day. Per staff, the substitute teacher closed the nap room doors leaving napping infants unattended. Staff further stated that there is a poster placed on both sides of the double doors that reads, " STOP! Before closing the door please make sure there is enough number of friends to teachers".
According to parents interviewed during the course of the investigation, they have no concerns regarding supervision. Parents stated that their children have not sustained any serious injuries or injuries that are out of the norm. for their infant children. Parents further stated that they have not observed sleeping infants left alone without adult supervision.
Based on the information obtained through observations and interviews, the allegation is substantiated. A substantiated finding means that the complaint is substantiated and the allegation is valid because the preponderance of the evidence standard has been met.
1 Type A deficiency is being cited on the attached LIC9099-D.
Upon receipt, the Licensee shall post the Notice of Site Visit and the Licensing report. This report and the Notice of Site Visit shall be posted for thirty (30) consecutive days. Failure to maintain posting as required will result in a $100 civil penalty. A copy of this report shall be provided to the parents/guardians of the children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parents/guardians of any children newly enrolled at the facility for the next twelve (12) months. The LIC 9224 Acknowledgement of Receipt of Licensing Reports must be maintained in each child's file immediately upon receipt from the parent.
A copy of this report, notice of site visit, and appeal rights were provided. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
A notice of site visit was given and must remain posted for 30 days.
