On 05/30/2025 Licensing Program Analyst (LPA) Brittany Lovest and Cristina Castellanos conducted an unannounced 3 year Required Inspection at Torrance Montessori School. LPAs met with Director Yanmei Long.
LPAs observed 34 children being supervised and cared for by 3 staff members. Facility hours of operation are Monday – Friday, 7:30am - 5:30pm.

LPAs toured the facility indoors and outdoors for a Health and Safety Inspection. Capacity and limitations as specified on the license are being maintained.

Upon arrival LPAs observed 34 children in care with two adult staff members providing care and supervision in the outdoor play area in the back, which poses/posed an immediate health, safety or personal rights risk to persons in care.

During the course of the inspection LPAs observed 16 children in care while S2 did not have visual supervision of all children in care on multiple occasions, which poses/posed an immediate health, safety or personal rights risk to persons in care are always under visual supervision of a teacher. Type A Deficiency cited. See LIC 809-D.

Furthermore, based on LPAs observations and record reviews staff S2 only has three (3) ECE units and providing direct supervision to 14 children independently, which poses/posed a potential health, safety or personal rights risk to persons in care. Type B Deficiency was cited. See LIC 809-D

There are no pools, ponds, Jacuzzis, spas or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPAs inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPAs reminded director to frequently check for furniture or equipment for loose or pointed parts and to discard or remove from areas accessible to children.

Teachers are responsible for the day-to-day cleaning, assist with cleaning and disinfecting of classrooms and outdoor space. All toilets and handwashing facilities are in safe and sanitary operating condition. Water temperature, toilet paper, paper towels, safety and sanitation were inspected floors in the facility are clean and safe.

There is no kitchen on the premises. Food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Facility provides snacks and families are responsible for providing meals in adequate packing, eg. Ice packs in lunch bags and/or thermos. Drinking water is available both indoors and outdoors. Children bring in their own water bottle and meals from home. LPAs observed snack menu posted.

Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has a centralize fire alarm and smoke detector system throughout the front classroom and the back Classroom. The facility has a fully charged 3A:40B:C Fire Extinguisher. LPA Lovest observed a working carbon monoxide detector in bathroom near entrance of Facility. LPAs reviewed the Disaster, earthquake and fire drill log. Fire Drill log was completed on 4/4/2025.

LPA Lovest reviewed 6 children files and observed the files to be in compliance including but not limited to current contact information of parents and authorized representatives, and authorization for medical treatment, signed Parent’s Rights and signed Personal Rights receipts. LPA Lovest reviewed 3 employee files. Based on record review, the licensee did not comply with the section cited above in 0 out of 3 staff members present during today's visit did not have a Valid CPR and First Aid certificate, which poses/posed a potential health, safety or personal rights risk to persons in care. Type B deficiency cited, See LIC 809-D. LPAs reminded Director that Mandated Reporter training, Child Care provider course is required, and certificates are to be renewed every 2 years following the date on which the initial or last training certificate was received.

LPAs reviewed records and Criminal Record Clearance for the adults who were present during today’s visit. All Adults present during today’s visit were Criminal Record Cleared.

LPA Lovest informed Director, Yanmei Long today’s report dates 5/30/2025 documents two Type A Citations. Director was reminded that Type A citations shall be posted for 30 consecutive days as there is/are immediate risk to the health and safety or personal rights of children in care. Also, LPA Lovest informed director to provide a copy of this licensing report dated 5/30/2025 that documents any Type A citation to parents/ guardians of all children currently enrolled by the next business day or the next day children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of the Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child’s file for verification.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The name of the childcare center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times.

Director Yanmei Long as informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Lovest reminded the staff to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Childcare Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, 4 Deficiencies were cited. Please see LIC 809D.

During the exit interview the report was reviewed, and a copy was provided to Director Yanemi Long.

LPA Lovest posted a Notice of Site Visit which must remain posted for 30 days.
