25 | On June 28,2024 at 9:30 am, Licensing Program Analyst (LPA) A. Wallin conducted a Case Management Incident inspection. This inspection is regarding an incident that took place on June 6, 2024 in which a child while playing on the playground, was discovered semi-responsive with a fever of 99.9. LPA met with director, Yolanda Lopez, who provided information and assistance during inspection. A census of 58 children and eight staff was taken. During inspection, LPA observed a census of 24 school age children and two staff present.
During the course of the inspection, LPA completed children and staff record review and conducted interviews with two children, three staff and one parent. During inspection, LPA evaluated outdoor play area where incident occurred. LPA observed age-appropriate playground area and toys for children in care. LPA observed outdoor play area is enclosed and no hazards present to children in care. LPA observed water play equipment, which consisted of four plastic circle wading pool containers with a quarter amount of water and some plastic water toys and a plastic tub container with water, soap and sensory toys such as: foam numbers, large plastic Lego blocks, plastic bugs, plastic dinosaurs, plastic shovels, etc. Per director, on day of incident, Orbeez balls were included in the water play activity tub. Per director, based on camera footage, it is assumed that child consumed some Orbeez balls during the water play. During investigation, LPA unable to review camera footage on day of incident, due to the footage being erased after a 7 day time period. Per director, since incident; Orbeez are no longer included in the water play sensory tub. During inspection, LPA did not observe any use of Orbeez balls in water play activity.
Per staff interview, three teachers were present on the playground, who maintained supervision during incident and provided immediate assistance. Per parent interview, according to doctor’s visit, child had marijuana substance in his system that led to incident. Per children, staff and parent interviews, it was not confirmed if child ingested any unfamiliar substance prior that led to incident.
