Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced Required Annual Inspection. This is an Infant Care Center which is located on the same grounds of the Preschool license #197493392 hours are from 7am to 6pm Monday through Friday. Upon arriving at the Infant facility LPA’ observed 6 infants being supervised in 2 classrooms one of the six infants was napping in crib. LPA observed sign/sign out sheets at entry door of the facility. Two teachers were observed in the Infant Center during inspection. Facility is operating within capacity limitations.

LPA Powell toured the facility, all identified rooms per facility sketch were inspected, the following was observed. Two rooms one main room for activities the second room for napping and changing of diapers there is a hallway between the rooms. Infant rooms were found to be clean and free from any potential hazards. LPA observed age-appropriate toys in classroom #1. Toys were inspected for safety, no loose parts, sharp edges or splinters. LPA observed infant changing station in room #2 to be in sanitary condition, sink available next to changing table with 1-inch-thick washable padding. Classroom #1 highchairs were observed which are also infant feeding table and activity table.. LPA observed separate sleeping area for infants with sufficient infant cribs in sleeping room #2. Mats are available for infants able to crawl out of cribs. All cribs were inspected and found in good repair, free of any hazards, and with adequate space allowing for entry/exit from napping space. Each crib, mat or cot is occupied by only one infant at a time and cribs are kept free from all loose articles including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care. Staff physically checks on sleeping infants every fifteen minutes. Documentation for infants up to 12 months including sleeping position was observed.

Individual Infant Sleeping Plan is completed and in file for each infant up to 12 months of age. Infants up to12 months are placed on their back for sleeping if not noted by parent on the Infant Sleeping Plan. Staff-infant ratio requirements are being met while infants are sleeping. LPA observed a small refrigerator in classroom #1. LPA observed food preparation area.. LPA observed labeled bottles and sippy cups with infants names in refrigerator. LPA observed Furniture was found to be in good repair and age appropriate. No baby walkers and bouncers are on the premises. There is adequate heating, lighting and ventilation. Drinking water is readily available in the classroom.

Infants have individual cubbies for diaper and extra clothes storage. Infant Daily logs are used for daily communication with parents. Director states that the isolation area for sick infants is located in the Director's office. Parents are notified when an infant is sick or ill.
All Cleaning supplies are out of reach of the children and is stored separately in classroom.

Inspection of the outdoor play area was conducted. Play equipment was inspected and found in good repair. Infants use the same play area as preschool but at different times. Drinking water is readily available for the infants during out door play. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves. LPA inspected area for hazards, safety and age-appropriate play equipment



LPA reviewed Infant files 2 of which did not have LIC701 completed during inspection and 2 staff files both teachers have Mandated Reporter on file.

The following was discussed with the licensee:
Mandatory Forms for the children’s files and provider’s files, Requirements for fire drills, earthquake drills and documentation for both. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

State law prohibits baby walkers, bouncy seats, exer-saucers and any other items that fall into that category.

All infants must be placed on their backs when sleeping to prevent S.I.D.S. (Sudden Infant Death Syndrome), and that the Provider is required to wash hands after every diaper change and to never shake a baby to prevent the Shaken Baby Syndrome

Requirements for fingerprint clearances and associations were discussed with the Director. Director can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12-month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

The director was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).

An appeal may be filed, in writing 15 business days from the date of receiving the penalty assessment. As of September 1, 2016, SB 792, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

The director was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on childcare licensing, and provide many other helpful resources to the licensees and the public. New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
