Licensing Program Analysts (LPAs) Lilia Hernandez and Roberto Luque Avila conducted an unannounced complaint inspection to the above facility on 07/17/2024. LPAs arrived at the facility at 9:20AM and met with Sade Williams, Center Director. LPAs singularly conducted a tour of the facility per Center Director’s directive. There were 12 children, and 1 staff upon arrival.

The purpose of the visit is to address deficiencies that were discovered during an inspection of a complaint investigation conducted by the Department.

Upon arrival, Center Director stated to LPAs that today was not a good day due to water play activity. LPAs reminded Center Director regarding Inspection of Authority. Center Director spoke to LPAs in a loud voice in front of children in care addressing the interviews that were conducted during a previous investigation.

At 11:50AM, LPA Luque Avila observed Staff #1 outdoors with 7 children. Children were engaged in a water activity. Children were playing in an inflatable water slide. (photo taken)

There shall be at least one adult who has a valid water-safety certificate on file at the center present during water activities.

A ratio of not less than one adult, including teachers, to every six children, or fraction thereof, shall be maintained during water activities in or near any of the bodies of water.

Inflatable water slide has an instruction label that indicates inflatable water slide of for children 3 years and above. There is also a warning indicating there should be a maximum of 3 players at a time for the whole product and maximum of 2 players at a time for the inflatable part. (photo taken) ---Page 1 of 2

Per Center Director, water safety ratios of 1 adult to every six children will be implemented immediately and regulation will be reviewed with staff. Meeting Agenda and sign in signatures of staff in attendance will be submitted via emial to LPA Henandez by POC due date.

Per Center Director, They may or may not have a certificate. Center Director was not able to provide proof of certificate to LPAs during inspection.

The following deficiencies listed on the attached deficiency pages are being cited in accordance with California Code of Regulations Title 22.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
