On October 2, 2024, at 10:06AM, an announced Case Management visit was conducted by Regional Manager (RM) Claudia Guangorena, Licensing Program Manager (LPM) Rita Ramos, Licensing Program Analyst (LPA) Lilia Hernandez for the purpose of the informal meeting requested by the licensee.

The purpose of the informal meeting was to discuss a recent case management deficiency inspection conducted on 07/17/2024 by LPAs Lilia Hernandez and Roberto Luque Avila.

Upon arrival, RM, LPM and LPA were greeted by Alyce Alford, Licensee and Sade Williams, Center Director. Licensee and Center Director guided licensing staff on a tour of the facility. There were 10 children in care and 1 staff present.

During the informal meeting, concerns regarding the complaint process were discussed with Center Director and Licensee. RM and LPM provided answers to questions that both Center Director and Licensee needed clarification on.

There were no deficiencies cited during today’s inspection.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview conducted and report was reviewed with the licensee, Alyce Alford. Appeal Rights were provided.
