On 08/18/2025, Licensing Program Analysts (LPA) Brittany Lovest conducted an unannounced Random/Annual Inspection for the above-named Infant Child Care Center. The facility has a preschool license on site 197493392. LPA entered facility and disclosed the purpose of the visit. Center is licensed for 7 infant children, ages 0 months through 24 months. LPA Lovest met with designated Director M.Cortez.
LPA Lovest toured the facility indoors and outdoors and observed 4 children in care, being supervised and cared for by 3 fingerprint cleared staff members. Facility hours of operation are Monday – Friday, 8:00am – 5:00pm.

Facility has an approved outdoor waiver.



Capacity and limitations as specified on the license are being maintained.
The CCC is in compliance with Title 22 regulations regarding Staff to Student ratios. LPA Lovest advised Director of the importance of sign in/sign out using the full first and last name of the parent/authorized representative and time of checking the child in and out daily.


Facility provides breakfast, lunch and snacks. LPA Lovest observed Menu posted in classroom. During the walk through of kitchen at approximately 9:30 am LPA Lovest observed 3 open food containers and 1 opened beverage container with a labeled expired date. Type B deficiency cited. See LIC 809D. Director discarded items and explained some food items were previously frozen. LPA advised Director to develop a plan of proper storage and labeling items to ensure storage and shelf life.

Director, Sade Williams arrived to the facility at 9:33am.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are no swimming pools, ponds, or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible to children in care housed in a locked cabinet under sink near activity room.

LPA Lovest observed a fully charged 2A:10B:C fire extinguisher in the hallway between the activity area and napping room. LPA Lovest observed working carbon monoxide and smoke detector. Pull Fire Alarm system was observed in activity room.

All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPA inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts.

At 11:01 am LPA Lovest observed an infant refrigerator and observed one unlabeled opened medicine container without documentation from authorized representative. Type B deficiency cited. See LIC 809D.


LPA reminded the facility to ensure that all medication is stored with all necessary LIC forms. All prescription and nonprescription medication shall be centrally stored in accordance with the following requirements: medication shall be kept in a safe place inaccessible to the children in care, each container shall have an unaltered label.

At 12:30 pm LPA Lovest observed 4 children in napping room without adult supervision. LPA Lovest observed teacher transitioning children from activity room into napping room one at a time, leaving children unattended which is an immediate risk to children in care. Type A deficiency cited. See LIC 809D.


LPA observed 4 cribs. Napping equipment does not block entrances or exits. Infant mattresses were firm with tightly fitted sheets. Cribs did not have any loose objects, bumpers, objects hanging, or objects attached. LPA reminded Director to continue following the Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, and provided PIN 20-24-CCP. LPA reviewed sleeping logs for children present during today’s visit.

LPA reviewed 4 children’s files and observed files to be in compliance with current contact information for authorized representatives and/or relatives who can assume responsibility for the child, immunization records, and authorization for medical treatment. LPA reviewed 4 Infants’ needs and services plans. LPA observed 4 of 4 Infants needs and services plan missing authorized representative and directors signatures. Type B deficiency cited. See LIC 809D. LPA reminded Licensee the infant needs and service plan should be updated regularly or as needed.

LPA Lovest reviewed 3 personnel files to be in compliance with required documentation. LPA reminded the Director that CPR/First Aid is to be EMSA approved and both CPR/First Aid and Mandated Reporter training are to be renewed every 2 years. LPA provided director with LIC311A and LIC125 to use as a reference when auditing files.

The director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.



LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended This facility provides Incidental Medical Services - IMS.

LPA Lovest reviewed storage of medication and equipment/supplies, and reviewed children's, personnel, and administrative records. For IMS information see PIN 22-02-CCP. LPA Lovest reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

LPA Lovest informed Sade Williams that this report dated 8/18/25 documents one Type A citation. Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Lovest informed the Director to provide a copy of this licensing report dated 8/18/25 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.


Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 1 Type A and 3 Type B citations were cited today. See LIC 809D.
