On 10/31/2024 Licensing Program Analyst (LPA), Tyra Chavies conducted an unannounced Annual Required Inspection to the facility to ensure the health and safety standards as required governing California Family Child Care Homes (FCCH). LPA observed 14 children in Care being supervised by 5 staff and Catherine’s husband Lawrence Mutuc.

Present at the time of inspection was Catherine Mutuc and Charmaine Mutuc, 3 staff members and Catherine husband Lawrence Mutuc. Licensee's states hours of operation for the FCCH are Monday -Friday 7:00 AM-5:00 PM. LPA Chavies verified the facility phone number is (310) 308-6851 and 424-744-6023.

LPA toured the home inside and outside. The home has 3 bedrooms, 2 bathrooms, a back yard, living room, kitchen, indoor patio and attached garage. Bedrooms 1 and 2, dining and living room, kitchen, bathroom 1, and garage will be inaccessible to children in care. The following areas will be accessible to children in care: Bedroom 3 (sleeping area0, bathroom 2, indoor patio (Sleeping area) and backyard (Play area). There are age-appropriate toys, books, and equipment for the children, at the time of inspection. During inspection, LPA observed that in Bedroom 3 and indoor patio children were all sleeping on cots. Licensee will ensure that children in care are being supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children.

There are no weapons or firearms on premises. Per the licensee, there are no bodies of water or pools on the premises. There are no pets on the premises.


The facility appears to be safe, sanitary and in good repair. All the cleaning products were stored and made inaccessible to the children. LPA Chavies reviewed storage of equipment and supplies. Outdoor play equipment is in good condition, free of any hazards and there are age-appropriate toys. The bathroom was toured. Licensee will ensure that children are being supervised at all time and/or make bathroom drawers inaccessible to children in care. The food preparation area and storage areas are clean and in sanitary condition. Drinking water is available both indoors and outdoors. The facility has a smoke and carbon monoxide detector. LPAs inspected and verified that there is a 2A10BC fire extinguisher. LPA reviewed 5 children's file and observed files are complete with all the necessary LIC forms. LPA reviewed 5 staff files including both licensee's. Licensee's Mandated Reporter Training, Pediatric First Aid and CPR were reviewed with valid certification and valid dates. LPA Chavies did observed LIC 503 missing from each employee.

POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall. Fire and disaster drills are being conducted as scheduled every six months.

· A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.

· Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

No smoking, No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into that category are not permitted in the facility.



When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN).

An exit interview was conducted with licensees, Catherine & Charmaine Mutuc

Per Title 22 of the California Code of Regulations, the following Technical Violations were discussed. (See LIC 9102)

A copy of this report was read and given to licensee as well as LIC 9213 (Notice of Site Visit form.) LIC 9213 is required to be posted for 30 days.
