25 | On 02/02/2032 @ 9:30 AM, Licensing Program Analyst (LPA) Miriam Cohen conducted a Case Management Unusual Incident inspection to follow up on the self-reported incident that occurred on 01/24/2023 at Tutor Time Learning Center located at 2848 Sepulveda Blvd., Torrance, CA 90505. The El Segundo Regional Office received the incident report on 01/25/2023. Upon arrival, LPA observed proper care and supervision. All center staff that was present during today’s inspection had fingerprint clearance and associated to the designated license number. LPA interviewed the following: Assistant director (Staff #1 or S1) and witnessed teacher (Staff #2 or S2). Per assistant director, the parent of the injured child (C1) informed the director that the child’s last day will be on Friday, 01/27/2023 due to personal reasons.
Based on the information that were gathered through interviews, record reviews, and visual inspection of the playground (site of the incident), it revealed that there was one adult supervising nine children in the daycare center on the day of the incident. During the interview, S2 revealed that she witnessed the incident and cared for C1. There were no other adult witnesses during the incident.
According to the narrative provided by S2, C1 was playing/swinging on the high hang bar, lost grip of the bar, fell on the ground, and landed on right shoulder. S2 was tying one of the children’s shoelace and heard a thump. S2 immediately picked up and assessed the child for any injuries. C1 cried and complained of right shoulder hurting. S2 provided, TLC and placed an ice pack on C1 right shoulder. The center notified parent of C1 and mother came to the school immediately. On 01/25/2023, the parent of C1 notified the school that C1 sustained a fracture and will be on a sling for at least six weeks. The parent and C1 came by the school on 01/27/2023 to bid goodbye to classmates and teachers.
At this time, based on the available information, it does not appear that this incident was the result of a Title 22 violation for lack of care and supervision.

An exit interview was conducted; the notice of site visit must be posted for 30 days upon receipt. |
