On 09/17/2024 Licensing Program Analysts (LPA) Cristina Castellanos and Judy Laureano arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegation. Upon arrival, LPAs met with Administrator Yohana Romero and discussed the purpose of the visit. Shortly after at approximately 10:00am Director Chanel Cunningham arrived at the facility.

Facility operates an infant license on the same location. LPAs toured the facility both indoors and outdoors. LPAs observed 12 infant children and 4 infant staff members providing care and supervision.

LPAs observed and inspected the preschool program and observed 35 children and 5 preschool staff members providing care and supervision. Present during today’s inspection was Administrator staff member Y. Romero, Director C. Cunnigham and the facility’s cook N. Newell.

LPAs requested the following documents: children's roster, staff roster, personnel records, injury reports for the month of September and the parent handbook.

At approximately 09:44am LPAs observed individual S. Santana in the preschool playground with 12 children.

Based on observation and record review it was confirmed that staff member S. Santana only has a Child Development Assistant Permit issued by CTC and six (6) Early Childhood Education units from Santa Monica Community College, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.



There was one deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information.

Upon receipt of this report, the Director shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Director was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

An exit interview was conducted and Plan of Correction was reviewed and developed with the Director. A copy of this report and appeal rights were discussed and left with the Director, whose signature on this form confirm receipt of these documents.

Director agrees to develop a Staffing Plan to ensure that the children in care are always supervised by Qualified Staff Members at all times. Director will submit the developed plan in writing to LPA via email by the POC due date.

Based on LPAs observations and record reviews, the facility did not comply with the section cited above staff S. Santana only having a Child Development Assistant Permit issued by CTC and six (6) ECE units from Santa Monica Community College, which poses/posed a potential health, safety or personal rights risk to persons in care.
