32 | At approximately 09:44am LPAs observed individual S. Santana in the preschool playground with 12 children.
Based on observation and record review it was confirmed that staff member S. Santana only has a Child Development Assistant Permit issued by CTC and six (6) Early Childhood Education units from Santa Monica Community College, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
There was one deficiency cited during today’s visit in accordance with the California Code of Regulations, Title 22, Division 12, and Chapter 1. See LIC 9099-D for additional information.
Upon receipt of this report, the Director shall post the Notice of Site Visit and any Licensing report documenting a Type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child’s file immediately upon receipt from parent. Director was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.
An exit interview was conducted and Plan of Correction was reviewed and developed with the Director. A copy of this report and appeal rights were discussed and left with the Director, whose signature on this form confirm receipt of these documents. |
