Licensing Program Analyst (LPA) Shandra Powell, conducted an unannounced required 1 year inspection to the above facility on 01/19/2023. LPA arrived at the facility, identified self and gave director the reason for the unannounced visit. LPA provided Director, Gibson with a copy of the LIC 125 Entrance Checklist. Director guided analyst on a tour of the facility inside and outside. This is a preschool program which consists of 4 classrooms. Facility operation hours are Monday through Friday from 6am to 5:30 pm. The facility also has a Infant Center located on the same grounds.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection Staff #1, #2 and #3 with 22 preschoolers. The facility was observed to be within the license capacity and limitations. All Documents to be posted in a public area were posted and viewed during inspection. All documents to be reviewed during inspection were reviewed and complete. LPA reviewed the Electronic Sign in and Out Sheets. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPA observed children being signed into the facility, however the parent and or guardians of the children are not signing some of the children in or out daily. This poses a potential health and safety risk to children in care.

LPA observed all children are under supervision, including visual supervision, of a teacher at all times. LPA reviewed 7 children’s files and observed files were complete. LPA reviewed all staff files present during this inspection and observed files were complete. LPA observed the menu posted monthly.

LPA observed age appropriate equipment in each classroom and observed furniture and equipment in good condition. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. LPA observed cots for napping. Director stated children linens are cleaned by the facility every Friday. LPA observed a washer and dryer in facility. The restroom is located inside of the classrooms. The 2 year old classroom has a restroom located inside of their classroom. All restroom equipment was inspected and was noted to be clean and equipped with good flushing toilets, soap dispenser, paper towels.

Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. LPA observed cleaning solutions on top of high cabinet during inspection. Carbon Monoxide and Smoke detector were observed and tested during inspection. LPA observed Fire Extinguisher and Fire Alarm. Fire Extinguisher was last serviced on 01/2022. Director stated the Fire Department has been notified of the yearly service needed and will come to facility in February 2023 to service both fire extinguishers. LPA reminded staff the Fire Extinguisher must be serviced once a year and or a new extinguisher can be purchased.

All kitchen areas/food preparation areas and food storage area are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids or solid waste bags shall be discarded immediately after each meal. LPA observed children cups labeled with children names. Drinking water is readily available for children inside classroom and outdoors. The facility provided Breakfast, Lunch and Snack. Snacks were reviewed for availability, quantity and appropriateness to children in care. Menus for the past 30 days are available upon request.

Outdoor playground equipment is free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slide, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard.

LPA advised that no children shall be left without the supervision of a teacher at any time. LPA reminded staff all children be supervised to and from the restroom at all times.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free there are no firearms stored on the premises. There are no pools or bodies of water at the facility.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children's roster was reviewed and is current. Disaster drill log was available last drill was conducted January 11th 2023.


Facility staff was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/ day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

First Aid supplies were observed in the area between the main office and kitchen. During inspection LPA observed and reviewed medication which 3 children receive Incidental Medical Services (IMS). All medication was up to date and Director stated the medication is located in a lock cabinet in the front office at all times. LPA advised Director to create a document regarding each child's IMS services provided. All expired medication must be given back to the child's parent/representative.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA and Director discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to: California Department of Social Services | Community Care Licensing Division | 300 N. Continental Blvd. Suite 290-A |El Segundo, CA 90245

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

The Director stated the facility will go back to manual sign-in sheets and also keeping the electronic sign in also. On Friday Jan 13th 2023 the facility implemented a charge of $5.00 for parents who do not sign child in and or out. Teachers will confirm sign in and out signatures on either manual sign in or electronic before accepting or releasing child as of Plan of Correction 01/19/2023.
