On 08/11 /2022, Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced complaint visit for the purpose of delivering the findings for the investigation regarding the allegation above. LPA met with (Assistant), (Darien Womack). LPA observed 9 children in care and 2 adults (cleared and associated).

On 05/25/2022, LPAs, Loyce Phillips and Doris Whitmore arrived at this facility for the purpose of initiating the complaint investigation. LPAs interviewed 1 staff and observed 10 children in care. LPAs also obtained Sign in Sheets, Daily Schedule & Facility Roster

On 06/03/2022, LPA Doris Whitmore conducted a subsequent visit to interview staff and children in care. LPA observed 7children in care and 3 staff. LPA interviewed 3 staff and 1 child.

Based on interviews with relevant parties, record review, and information collected throughout the investigation, the allegation is deemed UNSUBSTANTIATED. Unsubstantiated – A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

There are no deficiencies being cited during today’s visit. An exit interview was conducted. A copy of this report was provided to ( Assistant Darien Womack) along with Appeal Rights and LIC 9213 Notice of Site Visit.
