Licensing Program Analyst (LPA) Lilia Hernandez conducted an unannounced complaint inspection to the above facility on 03/06/2024. LPA arrived to the facility at 9:34AM and met with Crystal Thompson, Licensee, who guided LPA on a tour of the facility. There were 12 children with 2 staff upon arrival.

The purpose of the visit is to deliver findings for the above allegations.

During the investigation conducted by the LPA, interviews were conducted, records were reviewed, pictures were obtained, observations were conducted, and a copy of children's roster were obtained.

Information provided by the reporting indicates that staff do not provide adequate supervision, staff did not provide a safe environment for children, staff did not ensure that hazardous items were inaccessible to children in care, staff do not provide adequate food service, and staff do not keep the facility clean and sanitary.

Per Center Director, low ratios are maintained in the facility. Ratios in the facility are below the required 12:1 child teacher ratio. The facility implements ratios of 8:1 child teacher ratios. Center Director stated that supervision is provided, and children are within staff visual and audio supervision. Prior to the facility opening, an inspection is conducted both indoors and outdoors to ensure no items that are of hazard to children are accessible. Food service is provided daily to children in the form of family style serving. The facility discards any left over food after every meal.

Staff interviewed made no disclosures to support the allegations above. Staff disclosed ratios maintain at the facility are 8:1 children teacher ratios. Staff also disclosed that hazardous items are kept inaccessible to children, both indoor and outdoors is inspected daily before children arrive or play outdoors. Staff also disclosed that food is disposed of after every meal.

Based on the investigation conducted by the Department it has been determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

A copy of this report, appeal rights, and Notice of Site Visit was provided.
