On 3/22/2023 Licensing Program Analyst (LPA) Judy Laureano conducted an unannounced Annual Required Inspection at Little Scholars Academy Child Care located at 4168 W. Slauson Avenue, Los Angeles, CA 90043. LPA was greeted by Crystal Thompson, facility director. LPA toured the facility both indoors and outdoors and a census was taken.

The following was observed during the initial inspection, 4 children with 1 staff member, K. Dunlap and director, providing care and supervision. At approximately 10 am. staff member A. Baker arrived at facility. Toward the end of the inspection, 2 other children arrived at facility.

Facility operates Monday through Friday 8:00 a.m. to 6:00 p.m. Facility is not available for overnight care or weekend care. Facility is licensed to serve 20 preschool children ages 2 years old to entry to first grade. Facility has a dual license for school age# 197493692 with a max capacity of 10. Inspection was only conducted for the preschool license.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.

Medications are made inaccessible and stored in a locked box in the preschool room- Currently facility does not have any children taking medication.

Teachers do the day to day cleaning between activities and director completes facility cleaning and disinfecting. Disinfectants, cleaning solutions and other hazardous items are made inaccessible in the top shelf in the kitchen area.

LPA observed two preschool classrooms. LPA observed clear doors in the classroom and ½ barn like door in preschool bathroom. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe.

Facility provide meals and snacks and menu is provided to families. Facility participate in local food program. All food is protected against contamination and any contaminated food is discarded immediately. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin Solid waste storage containers have tight-fitting covers and are in good repair.

Drinking water is available both indoors and outdoors.

Areas around high climbing equipment have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility contracts with a Pest control vendor to ensure that facility stays pest free. LPA observed working fire extinguishers in the classrooms and kitchen area and working carbon and smoke detector in each classroom. First Aid kits were observed in the classrooms.

Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption.

Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director’s Pediatric CPR and First Aid was completed and will expire 7/2023. Mandated Reporter has been completed by director and staff. LPA reviewed staff’s file and one file was incomplete. LPA issued a technical violation for missing Notice of Employee Rights (LIC 9052).

LPA reviewed a sample of children’s files and observed all 6 files were complete. LPA provided facility with a copy of the LIC 311A and LIC 125 to use as a reference to ensure all files have current documents.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility currently using Learning Genie as an electronic sign in/out and hard copy is available for LPA’s review.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Facility representative, Crystal Thompson was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) currently IS NOT being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted and report was reviewed with the facility representative Crystal Thompson. Facility was provided with a copy of their appeal rights.
