On 8/25/2025, Licensing Program Analyst (LPA) Brittany Lovest arrived at the facility for the purpose of conducting a complaint investigation. Upon arrival LPA met with Director Jacquelyn Thompson and observed 13 children in care being supervised by 4 staff members.

At 9:14 am LPA Lovest observed C1, C2 in Room 2 sitting in chair with buckled strap attached around children lap restricting child’s movement while 7 other children were sitting in chair with no buckle strap. Director disclosed the children are in chairs to ensure sitting during circle time. Based on observation and interview the Facility did not comply with Personal Rights regulation, which poses an immediate health safety or personal rights to children in care. Type A deficiency cited see LIC 809D.
Based on the observation and interview during the investigation process there is a preponderance of evidence to prove the alleged violation did occur and the allegation can be SUBSTANTIATED.

The following deficiencies are cited per California Code of Regulations, Title 22, Division 12,Chapter 1

LPA Lovest informed Jacquelyn Thompson that this report dated 8/25/25 documents one Type A citation. Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.Also, LPA Lovest informed the Director to provide a copy of this licensing report dated 8/25 /25 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, 1 Type A citation was cited today. See LIC 9099D.

Appeals rights were discussed, Notice of Site Visit was given and to be posted for 30 days.

An exit interview was conducted, and a copy of this report was read and given to Jacquelyn Thompson.
