13 | On 3/19/2025, Licensing Program Analyst (LPA) Suzette Ornelas made an unannounced visit for
the purpose of conducting an initial investigation regarding the above allegations. Upon arrival, LPA was greeted and let into the facility by Lead Teacher, Maria Chavez to whom the reason for the inspection was announced. LPA toured the facility and observed 50 daycare children (17 toddlers) and 10 staff. School also has an infant license on the premises (197493822).
During the course of the investigation, LPA Ornelas made observations, obtained documentation in the form of children’s roster, childrens file, pictures of classroom, and conducted interviews with staff and parents.
-Pertaining to the allegation that – Staff does not provide adequate supervision resulting in day care child sustaining injuries.
According to the RP, Child 1 (C1) sustained multiple injuries due to lack of supervision while attending the day care.
