On 04/25/2024 at 4:05 PM, Licensing Program Analyst (LPA )Katrina Chicote conducted an Unannounced Required – 3 Year inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Natalia Parkhomovskaya, who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. There were six preschool age children present during today’s inspection. Per Facility Roster, there are currently 17 children enrolled, Licensee confirms roster is current. Per Licensee, the facility’s hours of operation are Monday thru Friday 8:00 AM to 6:00 PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 03/04/2024. LIC 126 was provided upon entry.

This is a single story home which consists of one bedroom and one bathroom, Kitchen, Daycare Room 1, and Daycare Room 2 which is detached converted garage. Areas used by the children include the Daycare Room 1 (located upon entry to the home), Bathroom 1 (located down hallway to the right), and Kitchen (located to the left of the home). Per Licensee, areas off limits to children and parents include Bedroom 1. Per Licensee, this Bedroom 1 is locked during operating hours, LPA discussed requirement to make this room off limits when children are in care at time of inspection.

All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPA observed AC wall units through out the home and a wall heater upon entry that is barricaded making it off limits to children in care. There is a working telephone maintained in the home. At 4:09 PM, LPA observed Daycare Room 1 to have child size circular tables and chairs, and a couch that were all well maintained at time of inspection. LPA observed a variety of age appropriate toys, free of loose and sharp parts stored in shelves: including puzzles, painting easel, large legos, and games. LPA observed a cubby for children's belongings available. At 4:15 PM, LPA observed Bathroom 1 to have operable equipment, adequate supplies, and general area safety and sanitation at time of inspection. cabinets to be free of hazards at time of inspection. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. At 4:17 PM, LPA observed cleaning compounds and detergents to be stored in hallway closet across from Bathroom 1 to have operable safety lock. The Licensee states that there are no poisons used to kill bugs/vermin in the home. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the age, size, and abilities of children in care.

Per Licensee, the children will have access to backyard area for outdoor play which is accessible through Kitchen. At 4:18 PM, LPA observed backyard has turf grass and adequate perimeter fencing through-out the yard. LPA observed a variety of age appropriate toys free of loose and sharp parts available for the children in the backyard such as bicycles, ride along toys, dramatic play equipment, and climbing ladder all maintained and all trees, shrubs, and plants are maintained at time of inspection. At 4:21PM, LPA observed Daycare Room 2, which is converted garage, to have child size table and chairs well maintained. LPA observed a variety of toys in Daycare Room 2 stored in shelves well maintained and books and other daycare supplies available.

Per Licensee, they provide food for children in care and use Daycare Room 1 for eating. During inspection, LPA observed children eating snack on tables in Daycare Room 1. Licensee states isolation area for sick children is in Daycare Room 1. LPA observed space large enough to keep child separate if needed.

LPA did not observe any pets at time of inspection and Licensee confirms she has no pets. Per Licensee, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPA observation did not see any bodies of water around the premises or firearms/weapons at time of inspection.

At 4:14 PM, LPA observed the valve on the required 2A 10BC fire extinguisher indicates fully charged with proof of purchase receipt for 07/08/2019. Licensee states this is her only fire extinguisher at time of inspection. LPA observed light on dual smoke and carbon monoxide detectors lighting green indicating to be in operable condition. Per Licensee, First Aid kit is kept in the Kitchen, LPA observed First Aid kit supplies.

At 5:10 PM, children records and required licensing forms were reviewed. LPA did not observe immunization records for 5 of 6 preschoolers files reviewed. LPA discussed required forms that were not observed at time of inspection.

The Licensee has current Pediatric First Aid and CPR valid until 11/19/2024. Proof of immunization against measles was readily available during today’s inspection but not TdaP and Influenza (or written declination for this only). Licensee's Assistant, who was providing care, did not have immunization records available for review at time of inspection. The Licensee has taken Mandated Reporter Training but not current, LPA observed training certificate completed on 07/10/2018 and Assistants did not have proof of completion of this training at time of inspection.

The following were discussed:
Incidental Medical Services (IMS)
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility.



LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage ahttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Volunteers are encouraged but not required to take the training. Website: http://www.mandatedreporterca.com/

Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit.

Megan’s Law - Family Child Care Homes

Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.
