13 | Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced complaint visit for the purpose of delivering findings for the above allegation(s). LPA met with Raquel Luna, Director, and explained the purpose of inspection. LPA toured the facility, the following census was taken: Infant room, 3 staff with 7 children. Toddler A/Room 114, 3 staff with 10 children. Toddler B/Room 116, 2 staff with 8 children.
During this investigation LPA interviewed reporting party, facility staff and witnesses, recorded observations and obtained records from the facility. LPA reviewed teacher logs that showed child/teacher ratios were followed on date od incident. Staff #1 Interviewed stated they observed child #1 fall but was unable to catch child. Although, Child #1 was injured while in care, there is not supporting evidence to show that the fall was due to staff neglect. In regard to the allegation that staff is not meeting child's diapering needs; witnessed interviews did not support the allegation. LPA observed individual bins with diapers, diapering log that documents when children are changes and was consistent with center policy of checking diapers every 2 hours. -----Page 1 Report Continues
