On 7/15/26 Licensing Program Analyst (LPA) Angela Luz, met with Director James Aoyama and conducted an unannounced Annual Required inspection.  Upon arrival, LPA observed Director in ratio with 3 infant children. LPA provided Entrance Checklist (LIC 125) and the facility was toured indoors and outdoors for a Health and Safety Inspection. Days and hours of operation are Monday to Friday 7:30AM-6:30PM. The facility practices Japanese language immersion. The facility has a preschool license with toddler component at the same address (197420084).

At 9AM LPA observed 2 staff with 8 infant children. Around 10:30AM LPA observed one child that was in the infant classroom was now in the toddler classroom. LPA spoke with Director about children staying with their enrolled age component. Director understands that even though the toddler component ages (18-36 months) overlap with both infant (0-24 months) and preschool (24 months - 5 years old) component ages, the purpose of the toddler component is to have older infants and younger preschoolers together who may not be ready to fully transition to the preschool classroom/s. Director will speak with enrolled families about which component their child is in, and when they will transition to the next class. Director understands that children should not go back and forth between components. LPA and Director discussed contents of AB 2131 (Mullin 2022) and further review is needed.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Infant classroom was inspected for age-appropriate furnishings, equipment, and adequate storage for children's belongings. LPA observed furniture and equipment are in good condition and free of sharp, loose, or pointed parents. LPA observed sufficient toys and play materials in good condition.

LPA observed 4 cribs in the sleeping area. LPA inquired about the number of cribs relative to the number of present children. Director informed infants in care have staggered nap times. Bedding is changed before use by another infant. LPA observed extra mats for older infants who may be able to climb out of a crib. Bedding is provided and washed daily by the facility. 15 minute sleep checks that notate any signs of distress are conducted and documented.

LPA observed 1 changing table within arm reach of a sink. LPA noted there were stacks of fabric on the changing table. Director informed they do not use the changing table anymore. They use a changing pad on the floor. Director stated it if safer to change infants on the floor so that they do not accidentally get left on the changing table. Later, LPA observed the changing table free of fabrics and staff stated they use the changing table.

LPA observed floors and surfaces were free of debris and hazards, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged fire extinguisher that is accessible throughout the facility. Cleaning supplies are kept in high shelves, inaccessible to children. The center is equipped with a fully stocked first-aid kit that is available in the classrooms. The isolation area for when a child is sick is an empty classroom.

The facility provides AM/PM snacks and lunch. All center provided food items are properly labeled & stored separately from cleaning supplies. Director informed that meals are prepared at the other location (23325 South Vermont Avenue Torrance, CA 90502) and delivered to this location. Menus are posted at least one week in advance, dated and available for review. Drinking water is accessible by children in the classrooms. LPAs observed the children’s water bottles are labeled with their name.

Forms of discipline used is comforting the child and talking with them. Director understands that children's personal rights should not be violated, including but not limited to, no corporal punishment, interference with eating, intimidation, or other actions of a punitive nature. Licensee understands that children are to be treated with dignity, receive safe, healthful, and comfortable accommodations.

The facility has a waiver for outdoor activity space. LPA observed the outdoor activity schedule is posted and adhered to. The outdoor play area is fully fenced and has shade available. All equipment and surfaces are free from hazards.  LPAs observed age-appropriate outdoor toys and materials for the children. LPA noted one water fountain and two sinks in the outdoor area. Per Director, there are no firearms or other weapons on the premises. There are no bodies of water, or free-standing water accessible to children.

All individuals subject to criminal record review have a clearance and have been associated to the facility. LPA reviewed the facility roster & personnel report and copies were obtained. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Licensee was reminded to ensure staff renew Mandated Reporter for Child Care Providers and Pediatric CPR/First Aid every two years. LPA observed all required postings in a publicly accessible area. LPA reviewed the sign in/out sheets and noted they were in compliance. Fire/disaster drills are recorded and performed at least every six months, last drill was on 6/1/26.

LPA reviewed 5 random children's files and 2 staff files during today's inspection. Children files were complete. Each infant under 12 months of age has an Individual Infant Sleeping Plan maintained in the file signed and dated by the infant’s authorized representative. Infant Needs and Services Plans are updated at least every 3 months. 1 of 2 staff files had an Mandated Reporter Training for Child Care Providers that expired in 5/2026.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.).

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee Director of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
