25 | An unannounced Case Management Inspection was conducted on this day by Licensing Program Analyst (LPA) Raul Navarro to address an Unusual Incident Report that was received in the licensing office on 05/04/23. LPA arrived at the facility at 12:50 pm and met with Licensees Ashley & Marva Nunes who guided LPA on a tour of facility. There were no children present during today's inspection.
On 04/17/23, Child #1 stopped coming to the facility.Per licensee, the mother never came to sign Crystal Stairs paper work. Per licensee, she called Crystal Stairs who advised licensee to write on the time sheets, "abandonment of child care".
LPA interviewed the Licensee. Per Licensee Ashley Nunes, child did was not terminated. Child was taken out of the day care by parent and stated they were going to enroll child into preschool. Parent did not sign the attendance sheets for the resource and referral program Crystal Stairs. Per Licensee, Crystal Stairs tried contacting Child's parent. Parent called Licensee a few weeks later and asked if they can sign the time sheets. Per Licensee, the time sheets had already been sent in to Crystal Stairs. Per Licensee, Parent did not tell Licensee that they were going to make a complaint. Per Licensee, they made an incident report in case mom decided to make a complaint.
Based on all information obtained on this date, and interviews conducted with Licensee, no follow-up is necessary regarding the incident. There were no deficiencies observed in regards to today's visit.
Exit interview was conducted with Licensee Ashley Nunes. A notice of sit visit was provided and must remain posted for 30 days. |
