13 | On 09/3/2025, Licensing Program Analyst (LPA) Doris Whitmore arrived at the above name home for the purpose of conducting an initial complaint investigation. Upon arrival, LPA met with the licensee Jenny Gandy and discussed the purpose of the visit. LPA toured the home and observed 12 children and 2 staff.
The Department conducted a full investigation, which included staff interviews, interviews with relevant parties and other agencies, as well as a record review which included documentation related to the allegations
LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Personal Rights - Licensee did not provide comfortable accommodations to children in care and Physical Plant- Licensee allows children to have access to unsafe equipment in the facility.Therefore, the allegations are deemed unsubstantiated. Meaning, although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred. No deficiencies cited.An exit interview was conducted, copy of this report was read, appeal rights along with Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days.
