On 3/13/2026, Licensing Program Analysts (LPAs) Amelia Morales and Nathanael Mooberry conducted an Unannounced Required- 3 Year Continuation  inspection at the above facility. LPAs were greeted and let into the facility by Teacher Jessica Garciarivas, Teacher Garciarivas informed LPAs that Director was not present due to illness. Teacher Garciavarias phoned Director, LPAs spoke with Director Mojabi. Director Mojabi gave consent for Teacher Garciavarias to sign the report. The LIC125 Entrance Checklist was provided to help facilitate the inspection. This is an infant center currently licensed for a capacity of  36 infant. Children's roster was reviewed and is current. Facilities operating hours are Monday through Friday 8:00 AM to 5:00 PM.

LPAs observed the following classroom ratios:
"Play Yard" off the entrance of the facility, LPAs observed 1 teacher and 2 teachers assistants with 4 infants.

"Middle Room" off the entrance of the facility, LPAs observed no children in care.

"Cot Room" off the "Middle Room", LPAs observed 1 teacher aide and  7 sleeping infants. no infants in care and no staff present.

"Back Room" to the left of the "Middle Room", LPAs observed no children in care.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

"Food Room" to the left of the "Back Room", LPAs observed no children in care.

"Crib Room" to the right of the "Back Room", LPAs observed 1 teacher, and 1 teachers assistant , and 9 infants.

LPA observed teacher child ratios in compliance and staff have the required background clearances at this time. LPA inspected the sign in/out sheets located in the lobby; all infants present were signed in.

LPAs observed the following postings in the office lobby: License, Daily Schedule, Personal Rights, Child Care Center Notification of Parents' Rights Poster, Emergency Disaster Plan and the Child Passenger Safety Law. The Disaster drill log was available; the last fire drill was conducted on 1/16/2026 and the last earthquake drill was run on 1/16/2026. LPAs observed the LIC 308 Designation of Responsibility posted on the parent board be updated, to reflect the new Designee.

A walk through of the facility was conducted, the space was found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. Play area has cushioning material to absorb falls.

The classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPA observed telephone service, heating, lighting and ventilation to be satisfactory. Napping area was observed to be separate from the indoor activity area.
The facility has 8 cribs and 5 mats for infant napping. Each crib/mat is occupied by only one infant at a time and cribs are kept free from all loose articles including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care. Staff physically checks on sleeping infants every fifteen minutes. This includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing.  The sheets are laundered every Friday by the facility.

The bathroom and toileting areas were inspected, facility is equipped with 3 changing tables, 1 toilet and 2 sinks. LPA observed changing tables, mats, toilet and sink to be in good repair. LPAs advised to move diaper changer within arm’s reach of the sink and was noted on the deficiency page. All areas were found to be clean and hazard free.
Per Teacher  Garciavarias the isolation area for sick children is in the "Middle Room" off the entrance of the facility. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible. Carbon monoxide detector was observed in the classroom and is operable.

Refrigerator used for food storage is clean and free of litter, rubbish, rodents, and/or any other vermin. Infant bottles were observed to be labeled. All waste containers were in a cabinet and inaccessible for children in care. First Aid supplies were observed to be complete.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. LPAs advised that no children shall be left without the supervision of a teacher at any time.

All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Six children’s files were reviewed. Inspection of required forms was made and documented on the LIC 857.  Children's files were not complete.

Documentation for infants up to 12 months includes sleeping position if it is other than on their back. LPAs also reviewed staff records. The review of Staff records was documented on the LIC 859.

The facility is equipped with a working smoke detector and carbon monoxide detector. LPA observed fire extinguishers (2A 10BC) mounted by the entrance of the facility. the facility with service date of 3/3/2026.

Facility Representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. CCC Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.
