13 | On 08/01/2023 at 9:11 a.m. Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced complaint investigation and met with the Administrator Imesha Siriwardana. LPA explained the purpose of the visit and to conduct interviews with staff and children. LPA obtained a copy of student incident report, ouch report, myca food menu, roster, daily roster, personnel report, unusual incident/injury report. LPA also was able to review five files. LPA toured the facility indoors and outdoors and observed twenty-one children and four teachers.
On 10/19/2023 at 1:30 p.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Director Maria Agustin. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 14 total children in care and 3Teachers.
The Department conducted a full investigation, which included staff interviews, interviews with relevant parties, as well as a record review which included documentation related to the allegations. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Personal Rights- Staff are not allowing daycare child’s therapist into the facility, Food Services- Staff are not providing nutritious foods, and for Reporting Requirements- Staff did not report unusual incident to daycare child’s parent. Although the allegations may have happened or are valid there is not a preponderance of evidence to prove the alleged violations did or did not occur. Therefore, the allegations are deemed unsubstantiated. An exit interview was conducted, a copy of this report, appeal rights along with Notice of Site Visit were provided.
