On 2/8/24, at 8:10AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced visit to this facility to deliver findings on the above-mentioned allegations of "Staff do not provide adequate supervision" and "Staff released child to unauthorized representative". Upon arrival, LPA met with Licensee Representative / Riyesha Siriwardhana, who guided LPA on a tour of the facility. There were 12 children with 2 staff observed present. LPA explained the purpose of today’s visit.

During the course of the investigation, interviews were conducted, copies of Children's / Staff Rosters were obtained and reviewed and pictures taken of the physical plant. Also, copies of documents were obtained and reviewed regarding Child 1 and Child 2 (Sign in/out sheet, Identification & Emergency Information sheet).

Per Reporting Party, staff do not provide adequate supervision and staff released child to unauthorized representative.

During the interview with the Licensee, it was disclosed that there have been no such incidents involving staff not providing adequate supervision to children or staff releasing a child to an unauthorized representative. Per the Licensee, the parents of Child 1 and 2 did not bring up any concerns regarding the above mentioned allegations. Child 1 and 2 stopped attending the program and there was no reason disclosed by the parents. There have been no complaints in the past from other parents regarding children being left unattended. According to the Licensee, the father was the individual who picked up both children (Child 1 and 2) during their first day at the center, on 1/24/23. The father is listed on the Identification and Emergency Information sheet for both children as "Father/Guardian". LPA verified this information to be true.

During the interview with the Director, the Director denied the allegations of staff not providing adequate supervision to children or staff releasing a child to an unauthorized representative. It was disclosed that on 10/24/23, at 5:29PM, the mother of Child 1 and 2 emailed the Director instructing that her "husband / the kids dad" will be picking up the children from the program and disclosed the husband's name. Upon receiving the email, the Director notified Staff 1 of the instructions provided by the childrens' mother because she had already left for the day. The Director also asked Staff 1 to verify the father's ID during pick up. The last day for the children at the program was on 11/9/23. The parents of Child 1 and 2 did not bring up any concerns regarding the above mentioned allegations. Child 1 and 2 stopped attending the program and there was no reason disclosed by the parents.

During the interview with Staff 1, Staff 1 denied the allegations of staff not providing adequate supervision to children or staff releasing a child to an unauthorized representative. It was disclosed that on 10/24/23, Staff 1 received a message from the Director stating that the father of Child 1 and 2 will be picking up the children from the program and the Director asked Staff 1 to verify the father's ID during pick up. Per Staff 1, when the father arrived to pick up the children, Staff 1 verified the ID to match the name provided by the Director. It was also disclosed that when the father arrived to pick up the children, both children (Child 1 and 2) ran to the father, screaming "daddy, daddy". There were 4 children at the program at the time and none of the children were left unsupervised.

Parents interviewed did not present concerns related to the above-mentioned allegations and were pleased with the services and care being provided to their children.

Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.

The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Director / Maria Agustin Palncar and Appeals Rights provided.
