25 | On 06/19/2024 Licensing Prgram Ananyst( LPA ) Whitmore conducted an unannounced visit to follow up on an incident that occurred on 05/13/2024. LPA Whitmore met with the Site Lead Ivette Leon. The total number of children were 7 and 11 staff.LPA Whitmore reviewed the following documents from Behavioral Health Services Family Health Center, Individualized Nutrition/ Health Care Plan. and initial Health History. Child came from 1st Steps Early Headstart Program 07/12/2023-09/16/2023.Child enrolled at PACE Aloha 10/31/2023 under Headstart. Reviewing the Individualized Nutrition/ Health Care Plan was based on the mother's concern that student pockets food in her cheeks. small pieces/ chopped diet required to aid with chewing/ swallowing. During lunch time Site Lead sat with the child. Ms. Nulveia, the kitchen aid would chop the food. The child's food would come aside from the rest of the children. The child would notice the difference and push the food away. The child would not eat the chopped food.During the enrollment process on the Health History Forms mother stated that child needs food protein only chopped. Mother brought to the school an updated doctor's note that states that the child is able to eat regular solid foods, without cutting to a very small pieces, please feed according to her age. The doctor contacted the parent on 05/29/2024 to inform her that the letter was ready. Parent brought the letter to the school the same day she picked it up.
Based upon reviewing all the documentation that was provided there are no deficiencies. An exit interview was conducted and a copy of this report along with the Notice of Site Visit was issued to Ivette Leon Site Lead. |
