On 02/05/2025 at 1:02 p.m.Licensing Program Analyst (LPA)Doris Whitmore conducted an unannounced visit for conducting a Case Management Inspection for an incident that occurred on 01/21/2025 & 01/27/2025. and was reported to the Regional Office. LPA met with Regional Site Director Nudia Juarez and informed the nature of the visit. At the time of the visit there was 50 children and 12 teachers.

According to the UIR there have been two positive cases of Head Lice. The first lice case occurred on 01/21/2025 at 1:40p.m. The second case of lice was discovered today at 10:30a.m. The parents were notified when outbreak occurred, and the children were confirmed lice were picked up by representative. Exposure notices were sent to the Aloha Classroom parents, after the occurrence of lice on 01/21/2025 and on 01/27/2025. The facilities team will organize a deep cleaning.

LPA Whitmore interviewed the teachers of Classroom 4 (S1) and (S2). The Regional Site Director (S3) was interviewed as well. LPA Whitmore received the documentation that was given to the parents. The following documents were reviewed and obtained: Lice Outbreak Checklist, Exposure Notice, Information on Head lice. All children that were on the LIC 624 Form (Unusual Incident/ Injury Report has returned to school. All the children returned without a doctors note. During the interviews it was stated that the parents did a lice treatment at home. As of today 02/05/2025, there has not been any more cases of Head lice. Teachers sanitized the classroom, and the carpets were cleaned. During the interview(S3) stated that when the children returned to school, before the parents signed the child in, the teachers did a head check.

An exit interview was conducted notice of site visit and appeal rights were issued to Nubia Juarez, Regional Site Director. LPA Whitmore informed Nubia Juarez that the Notice of Site Visit must be posted for 30 days. Notice of Site Visit was posted before LPA Whitmore left the facility.
