On 7/16/2024 Licensing Program Analyst (LPA) Judy Laureano arrived at above mentioned facility for the purpose of investigating the above-mentioned allegations. LPA was greeted by Amy Mach, facility director and discussed the purpose of the visit. LPA toured the facility and observed 23 children in care with 8 staff members providing care and supervision. LPA initiated interviews.
LPA requested and reviewed children's roster, staff roster, staff time sheets for June 2024. Any documents not available during today’s inspection, director agrees to email LPA.
On 10/2/2024 LPA Laureano concluded all investigative interviews with all relevant parties. Based on LPAs observation, investigative interviews with all relevant parties, no information was disclosed that facility was violating children's personal rights by not ensuring children are kept in dry clothing. No information was disclosed that facility is operating out of ratio. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove or disapprove the allegations, therefore the allegations are found to be UNSUBSTANTIATED.
Upon on receipt of this report, the Director shall post the Notice of Site Visit. The Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty.
Exit interview was conducted and a copy of the report was provided to facility director, Amy Mach.
