On 07/02/2025, Licensing Program Analyst (LPA) Lisa Clayton arrived at The Montessori Preschool to conduct the Required 3-year Inspection, and to change the license status from inactive to active. LPA Clayton met with Assistant Director Virginia Wikkramatillake. There are no children enrolled or in care. Days and hours of operation are Monday- Friday from 8:30 a.m. to 5p.m. The CCC provides snacks and water. The CCC will provide care to preschool children ages 2 – 6 years old.

LPA Clayton toured the facility indoors and outdoors for a Health and Safety Inspection.

This is a two-story facility located on the premises of the Village Faith Foursquare Church.

The CCC is operating out of 2 classrooms, a children’s bathroom and outdoor play area only. The Reading/Activity/Napping Room (classroom #1) entry is through the church parking lot/parent entrance from the street, and adjacent to the Main Class (classroom #2). The Main classroom is adjacent to the church offices and the church HUB (social area, kitchen, adult restroom), and the children’s bathroom. The children’s bathroom is also adjacent to the church Hub through another door. The church Hub is adjacent to the Sanctuary and allows to the second floor of the building. The Hub (church social area and kitchen), the church offices and the entire second floor of the building are off limits to children in care. Off limit areas are to be made inaccessible to children in care by locked doors and visual supervision. All children are to be under the visual supervision of a teacher at all times.

There are no pools, ponds, or any other bodies of water on the property. There are no firearms or ammunition allowed or stored on the premises. All disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible, and kept in a locked cabinet area.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA Clayton inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment in the classrooms are in good condition, free of sharp, loose, or pointed parts.

LPA inspected the children’s bathroom and observed 1 sink and 2 toilets available for children’s use, which will accommodate 15 children. The sinks and toilets are in safe and sanitary operating condition, and floors in the facility are clean and safe.

Measurements for the indoor activity space for preschool was 1619.20 square feet, which will accommodate the applicant’s request. The gate surrounding outdoor play yard is 4 foot or higher. Age-appropriate toys and equipment were observed in good condition.

LPA Clayton conducted a Health and Safety inspection of the food storage area and observed it to be free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is to be discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. LPA Clayton observed children’s allergies posted on the refrigerator.

LPA Clayton observed a 2A10BC Fire Extinguisher in the Main classroom. The CCC has a Pull Fire alarm system in the church office hallway with a pull located in classroom #2. LPA Clayton reminded Director Virginia of the importance of conducting Emergency Drills and using a log sheet to document when the drills are being done. The log is to be made available to the department upon request.

There were no files available for review. The CCC has been inactive since November 2021 and those files are in storage. LPA Clayton provided Director Virginia with the LIC 311A (2/22) - Records To Be Maintained At The Facility - Child Care Centers, Infant Centers, School-Age Centers.

There are no staff at the CCC at this time. LPA Clayton reminded Director that all staff are to be in compliance with current CPR/First Aid, Mandated Reporter training certificates, health screenings, medical training, employee rights, transcripts and immunization records for childcare workers.

LPA Clayton reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Childcare Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA Clayton informed Director of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA Clayton reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Per Director Virginia prior to opening the following will be completed:

· Installed carbon monoxide/smoke detectors in the classroom(s)

· awnings/shade covering the play yard will be installed

· anchor the “slide” to the ground in the play yard

· 1 or 2 more children’s tables in the play yard

· Widen the ground cover/cushion on the play yard

LPA Clayton instructed Director to contact the department prior to reopening, so the above mentioned can be inspected.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, no Deficiencies or Technical Violations were cited.

An exit interview was conducted, and the report was reviewed and provided to Director Virginia. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
