On 11/16/2022 at 12:35 AM Licensing Program Analyst (LPA) Antonio Almanza, conducted an unannounced Annual Required Inspection for the preschool license. LPA met with Director, Sara Poage, and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday, 6:30 AM to 6:00PM. LPA verified the facility phone number is 323-937-7827.

The facility is on a compound composed of three buildings with different address located between 20th and 21st street on La Brea Ave. The main office and Bees Classroom (3 year old’s) and the Kitchen is located at 4946 W 20th St, Los Angeles Ca 90016.

To the right-hand side of the main building there is 4950 W 20th St, Los Angeles Ca 90016. Within this building there is the Turtle Classroom (2 year old), Elephants (older 2 year old and younger 3 year old), and the toddler classroom (18-36 months).

The third building is located at 5005 W 21th St, Los Angeles Ca 90016. Within this building there is the Super Star classroom(TK), Giraffs classroom (Pre K), and Owels classroom (Younger 3 year old children).

All classroom in the facility are equipped with First Aid Kit, fire Extinguisher (serviced October 13, 2022) and operating Smoke/Carbon Monoxide detectors. There is a child and staff bathroom in each building. There is is a changing table and sink within arms reach inside the Toddler bathroom. Medication are kept in the main office of the facility inaccessible to children in care. The Main office is the Isolation room in case a child becomes ill while in care and the staff bathroom will be used for children in Isolation.

There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons/cleaning supplies are kept in a locked storage area next to the kitchen.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls.

Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Capacity and limitations as specified on the license are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The facility is utilizing Procare Online application to physically sign children in and out of the facility using their full legal signature. Facility maintains a ratio of one teacher supervising no more than 12 preschool children and one teacher supervising no more than 6 Toddler children in care. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. Menus are posted at least one week in advance where an authorized representative can view them.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


Incidental Medical Services (IMS) are currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. A notice of site visit was given and must remain posted for 30 days.
