On 04/30/2026 Licensing Program Analyst (LPA) Doris Whitmore arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegations. Upon arrival, LPA met with Site Supervisor, Norma Izcuar and discussed the purpose of the visit. LPA toured the facility and observed 41 children in care with 10 staff members providing care and supervision. LPA conducted staff and children’s interviews. The following documents were obtained Sanitizing with Bleach Solution Chart, Site Cleaning Monitoring Tool, Restroom Cleaning Checklist, Washing & Sanitizing Procedure, Receipt of Water Cooler Service, Teacher's Daily Health and Safety Checklist, Work Order and Master List of Participants. Classroom Observations.

On 05/20/2026 Licensing Program Analyst (LPA) Doris Whitmore arrived at the above-mentioned facility for the purpose of investigating the above-mentioned allegation. Upon arrival, LPA met with Site Supervisor, Norma Izucar and discussed the purpose of the visit to continue with staff interviews. LPA toured the facility indoors and outdoors and observed 43 children in care. There were 10 staff providing care and supervision.

parties and other agencies, as well as a record review which included documentation related to the allegations. LPA did not observe nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of Physical Plant - Facility does not have hot water, Staff do not ensure facility is clean and sanitary, and facility is not providing a safe environment for children in care. The facility does not prepare food for consumption, but does clean the dishes and utensils used during meal time with a sanitizing solution, which is an alternative method. The facility was able to provide documentation showing that actions were taken to have the hot water heater repaired or replaced and alternative methods of sanitizing were being used to maintain a safe environment for the children in care.

Therefore, the allegations are deemed unsubstantiated. Meaning, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred. No deficiencies cited.
An exit interview was conducted, copy of this report was reviewed and issued along with appeal rights and Notice of Site Visit. Notice of Site Visit is required to be posted for 30 days.
