25 | On 2/2/24, at 1PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an unannounced Case Management Visit to this facility. Upon arrival, LPA met with Director / Christina Lechner, who guided LPA on a tour of the facility. There were 17 children with 8 staff observed present. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to the Community Care Licensing (CCL) on 1/24/24.
On 1/23/24, at approximately 11:45AM, during transition from the Infant 1 playground to the classroom, Staff did not conduct a proper name-to-face roll call, nor a sweep of the playground, resulting in Child 1 being left on the playground unsupervised between 30 seconds to 4 minutes. After entering the classroom and settling the children, staff quickly realized that Child 1 was not in the room and Staff 1 went immediately to the yard to get Child 1. Child 1 was found alone playing in the yard.
