On 11/19/2021, Licensing Program Analyst (LPA) Lillian Casillas conducted an unannounced Annual Required Inspection and met with Assistant, Socorro Villafan-Caldera, who stated Licensee, Marie Villafan, was at an orthodontist appointment. Licensee arrived at approximately 10:22AM. Days and hours of operation are Monday through Friday, 7:30AM to 5:30PM.

LPA toured the inside and outside of the single-story home with 3 bedrooms, 2 bathrooms, living room, kitchen, dining room, front yard, backyard, laundry room, and garage. An Entrance checklist (LIC 126) was provided to Assistant at the start of today’s inspection. Current facility sketch reviewed, and Licensee confirmed that living room, dining room, bathroom 1, bedroom 1, bedroom 2, laundry room, and backyard are the main childcare areas. Off-limits areas are: bedroom 3, bathroom 2, front yard, and garage. Licensee agreed to send an updated LIC999A to reflect that the garage is now off-limits. The garage was on-limits during the pre-licensing inspection on 7/25/2019. Licensee agrees to contact CCL with any new changes to the facility sketch in the future and submit an updated LIC999A.

Indoor
LPA entered the home through the front door that leads into the living room. In the living room, LPA observed 1 couch and age-appropriate toys. There is a fireplace in the living room, which is barricaded off by a storage unit. There are two wall heaters in the living room and in the hallway that are barricaded off with screens. In bathroom 1, LPA did not observe any medications, toxins, or cleaning compounds that would pose a risk to children in care. Bedroom 1 is the infant napping room. LPA observed two cribs, three floor mats, a changing table, and a camera. In bedroom 2, LPA observed 1 twin bed.

Indoor Off-Limits
LPA observed two child safety gates blocking entrance into the kitchen from the living room and hallway. In the kitchen, LPA observed a child safety latch on the drawer with knives. Bedroom 3 is made inaccessible by a child safety knob. Bathroom 2 is located inside bedroom 3. LPA observed child safety latches on the cabinet doors built-in the hallway.

Outdoor
The outdoor play area is located in the backyard of the home. The outdoor play area is fully fenced in and is free from defects and dangerous conditions. There is a covered patio area with two wooden children's table and chairs where the children eat snacks and meals. The reading nook is also located in the covered patio area. In the grassy area, LPA observed age-appropriate toys, a small wooden slide, two toy kitchens, a garden, and a hand washing station. LPA observed a shed that is made inaccessible by a child safety gate. The laundry room is adjacent to the garage and it is divided into two areas: washer/dryer area and diaper changing area. LPA observed a diaper changing table and a storage unit with diapers and wipes. LPA observed a child safety gate blocking off the washer/dryer area from the diaper changing area.

Outdoor off-limits
There are two side gates located on each side of the home that are closed and locked during operation hours. There is also a child safety gate secured to the side door of the detached garage, which blocks off the laundry room from the garage. Licensee stated that the garage is locked and closed during operation hours. There is no swimming pool or other bodies of water on the premises.

There are no firearms or ammunition on the premises. Detergents, cleaning compounds, medication and other hazardous items are made inaccessible. The first-aid kit and earthquake emergency kit are located in the laundry room. There are no stairs in this home. There is a 2A10:BC fire extinguisher (serviced 3/24/2021) in the kitchen, a functioning smoke detector and carbon monoxide detector in the hallway, and a fire alarm in the kitchen. Facility conducts routine fire drills.

There are currently 4 infants in care. LPA observed completed LIC 9227 forms for each as well as sleeping logs for each infant. LPA discussed Safe Sleep regulations with Licensee.

LPA reviewed all currently enrolled children's files and all staff files. Licensee’s Mandated Reporter Training is current. Licensee’s pediatric CPR/First Aid expires on 8/6/2023. Assistant's Mandated Reporter Training is current, and Assistant's pediatric CPR/First Aid expires on 2/15/2022. There are no excluded individuals present at this home.

Licensee has a small dog with all vaccines. Licensee stated that the dog remains inside bedroom 3 during hours of operation. When children are inside, the dog is moved outdoors.

Criminal Record Statement
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
