On 06/01/2023 Licensing Program Analyst (LPA), Lisa Clayton, arrived at the home un-announced, to conduct an Annual Inspection and was met by Licensee Cristian Corona. LPA Clayton observed 14 children being cared for and supervised by licensee and 2 staff members Operating hours are Monday – Friday, 8am – 5pm. Licensee provides lunch, dinner, and am/pm snacks.

LPA Clayton toured the inside and outside of the home for a Health and Safety inspection. The home consists of the following: 3 bedrooms, 2 bathrooms, living room, kitchen/dining area, family room, attached garage, and fenced back and front yard.

The ON LIMIT areas are as follows: the living room (napping area), family room (day care eating/activity/napping room), bathroom #2 and the fenced front yard and fenced backyard.

The OFF-LIMIT areas are as follows: bedrooms #1, #2 and #3, bathroom #1, kitchen, and attached garage all of which are made inaccessible by locked doors, child safety gates, and supervision.

The home has working smoke detector/ carbon monoxide combo, a working smoke detector, and a fully charged 2A;10B;C Fire Extinguisher. The home has adequate heating and ventilation for safety and comfort. LPA Clayton observed toys and play equipment that are safe. The home has working telephone service and LPA confirmed the phone number is (323) 359-8835.

Licensee called parents and had 2 of the 5 infants picked up immediately. LPA Clayton reminded Licensee of the Max Capacity as stated on her license. Licensee acknowledged understanding.

LPA Clayton reviewed children’s files and instructed Licensee to update the files for accuracy and compliance with Title 22 regulations. LPA Clayton provided licensee with the LIC 311 (Forms/Records to be kept in Your Family / Child Care Home), and discussed the documents required.

A review of facility records indicates that the Licensees Mandated Reporter training expires June 2024, and her CPR First Aid expires September 2023. Licensee is to provide a copy of her assistant’s MMR, Tdap, Tb and Flu vaccination or immunization records prior to returning to the FCCH.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of 100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA Clayton reminded licensee of the safe sleep regulations and discussed the Child Care Licensing Safe Sleep web page at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) are not being provided at this time. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Clayton provided licensee with an IMS Plan sample. Prior to making alterations or additions to a family childcare home or grounds, the license shall notify the Department of the proposed changes, including, but not limited to, the following:


· Conversion of a garage (either attached or detached) into a "childcare" room.
· Room additions to the family childcare home.
· Installation of in-ground or above-ground swimming pools, spas, fishponds, decorative water feature, fountains or other bodies of water.
· Construction of exterior decks or porches.
· Construction of play equipment including swing sets/climbing structures.
· Any change from an area of the family childcare home previously identified as "off limits" to an area where care and supervision will be provided to children in care.
· The licensee shall provide the Department with a copy of an inspection report when an inspection is required by the local building inspector as a result of the alteration, addition, or construction.

During the inspection LPA Clayton observed 5 infants in care. As specified on the Childcare License “MAX CAP (WHEN THERE IS AN ASSISTANT PRESENT: 12 – NO MORE THAN 4 INFANTS CAP 14 – NO MORE THAN 3 INFANTS. 1 CHILD IN KINDERGARTEN OR ELEMENTARY SCHOOL AND 1 CHILD AT LEAST 6 YEARS OLD”.

LPA Clayton also observed laundry detergent, fabric softener, and bleach cleaning wipes in bathroom #2 (on-limit to children in care).

Deficiencies and Technical Violations were cited today, Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and Health & Safety codes (see LIC 809D) and TV notes.

LPA Clayton reviewed the Deficiencies and Technical Violations with licensee. Licensee acknowledged understanding.

An exit interview was conducted, a copy of this report was read and provided to the Licensee.

LIC 9213 Notice of Site Visit was provided and required to be posted for 30 days.
