On 07/11/2024 Licensing Program Analyst (LPA) Lisa Clayton and LPA Ranita Richmond arrived at the facility to conduct a Plan of Correction visit and was met by Licensee Albina. LPA observed 11 children in care being supervised and cared for by licensee and 2 fingerprint cleared assistants.

On 07/03/2024, Licensee was cited for the following:
1. Licensee will ensure that each child has a file including all of the Title 22 required documents.
2. Licensee will conduct fire drills and ensure the drill log is made available during Department's request.
3. Licensee will ensure that all detergents, cleaning compounds, etc. are inaccessible to children in care.
4. The home shall be kept clean and orderly, with heating and ventilation for safety and comfort.

During visit LPA Clayton and LPA Richmond observed the following:
1. LPA’s observed a lock on the kitchen cabinet making cleaning products and detergents inaccessible to children in care.
2. On 07/09/2024 licensee emailed LPA Clayton a report from the contracted pest control company indicating a recent inspection and clean-up. LPA’s observed the cabinet under the bathroom sink to be clean and free of mouse droppings, and the refrigerator to be clean and orderly.
3. LPA Richmond observed the disaster drill log posted showing a recently conducted drill.

3 of 4 Citations issued on 7/03/2024 has been cleared.

An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Licensee.
