25 | On 2/20/26, at 8:45AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual/Random inspection to the above facility. Upon arrival, LPA met with Licensee / Sarah Eisen, who assisted wit the visit. LPAs disclosed the purpose of the visit. Entrance Checklist – Family Child Care Homes was provided to the Licensee. Licensee guided the LPAs on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present were Licensee’s husband / Moshe Eisen and Licensee's Assistant / Catalina Bollo. Individuals residing in the home are 2 adults and 6 minor children. There were 9 day care children present during today’s inspection, including one child who is the Licensee's own. Licensee states that there are currently 9 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Thursday, from 8:30AM - 2:45PM and Friday from 8:30AM - 12:30PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 2/20/26.
This is a single-story home that is part of a two unit building. The unit includes three bedrooms, two bathrooms, a living room, dining room, kitchen, laundry room, detached garage, and both a front and back yard. The second unit on the property is located directly adjacent to the Licensee’s residence at 606 N Poinsettia Pl. This unit functions as an independent living unit. According to the Licensee, the residents living in this unit do not use the backyard during the facility’s operational hours. According to the Licensee, parents drop off and pick up their children at the main entrance, which leads to the living room. Areas that are accessible to children are as follows: Living room, dining room, kitchen, bedrooms 1 and 3, bathroom 1, and the back yard. The home does not have a fire place. The back yard has age appropriate toys. Per Licensee, off-limits areas are as follows: Bedroom 2, bathroom 2, laundry room, the front yard and the detached garage (located behind the home). The off-limits rooms are made inaccessible by child proof door knobs/locks.
